			<content:encoded><![CDATA[<p><strong>Q.</strong> Dear Rob, I am a 17 year old who used to wear braces about 2 years back.I wore retainers for a year (day and night) but after that I stopped wearing it. Recently I noticed my wisdom tooth starting to grow and I feel my front two teeth have come forward a bit. Is this possible? The rest of my teeth are strait but the front two teeth are a bit forward-unlike how it was before. How do I push it back again? Do I need braces? If so do I have to take off teeth again to move it back?</p>
<p><strong>A.</strong>
<p>Hi&nbsp;the teeth can crowd at the same time that <a href="http://www.cosmeticdentistryguide.co.uk/articles/wisdom-teeth.html">wisdom teeth</a> appear but it is less clear whether its caused by the <a href="http://www.cosmeticdentistryguide.co.uk/articles/wisdom-teeth.html">wisdom teeth</a>. You could start using a retainer again. Reversing the effect would need a thorough clinical assesment before answering.<br />Regards Rob</p>

<p><strong>A.</strong> Uncontrolled Diabetes is a contraindication to surgery and does adversley affect the predictability of <a href="http://www.cosmeticdentistryguide.co.uk/implants.html">dental implants</a>. care should be taken if you lost your teeth due to gum problems.<br />Most of the main systems have &#8220;Bioactive&#8221; sufaces like Osseospeed (ASTRAtech) SLA active (straumann) and Nobelbiocare, Biomet etc, I mainly use Astratech <a href="http://www.cosmeticdentistryguide.co.uk/implants.html">dental implants</a> because it is proven to maintain the surrounding bone in later years.<br />Rob</p>

			<content:encoded><![CDATA[<p><strong>Q.</strong> I very recently attempted at a local dentists office to have my canine tooth (which had bonding on it) re-bonded. I wanted it to be a solid color and shaped appropriately without harming my gum. The dentist was highly unsuccessful. The bonding looks varigated (two colors) and was either too large (looked like a dog tooth) and now too small/short. Question: should it not be simple to mix a color to match the tooth and not be varigated? Also, I had one hour whitening done and it didn&#8217;t really whiten my teeth very well. It has been less than two weeks and what whitening occurred is going away quickly. I asked if I had enough enamel for the treatment (I am 51) and did not get a reply. Any suggestions on how to get my teeth very white and a my canine to match? I do take good care of my teeth, brush, floss, etc daily FYI. Thank you!</p>
<p><strong>A.</strong>
<p>Start with sudy casts of your teeth then modify the cast to the shape that you want balanced with potential conflicts in the Occlusion / bite. Then veneer the teeth with an opaque ( normally Porcelain) Veneer. AN assesment is needed to find out if a single veneer or the visible smile (?? 8 teeth) is needed. <br />Contact us for more details.<br />Rob</p>

Also, the gum above one of the crowns protrudes a bit more than its neighbor. That developed after the crowns were replaced a few years ago. My doctor has x-rayed and it doesn&#8217;t show an infection. I&#8217;m guessing a lot can go wrong with old dental work? Any advice would be appreciated</p>
<p><strong>A.</strong>
<p>Discoloured Root filled teeth can be a problem to shade match All ceramic Zirconia cores are a good route but the level of opaque will need to be judged by trial and error it usually requires a few visits. The crown margins should be about 1mm below the gum line to hide the dark root emerging. If this is not possible then some periodontal plastic surgery&nbsp; to reshape the gum using grafts&nbsp; / emdogain etc might be an option.<br />Rob</p>

<p><strong>A.</strong> contact your local primary Healthcare Trust (PCT) ask for Dental Contracts and they will tell you who has a NHS <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a> contract in your area. For Private treatment search for practices offering IV <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a>.<br /><a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">Sedation</a> is probably your best route which is why I given those details although you need to speak to the provider to wexplain what this be be like in your case. The risks associated with general anaesthetics mean it is rarely indicated for dentistry, and if it is you would need a hospital refferal from a dental practice like the <a href="http://www.cosmeticdentistryguide.co.uk/articles/sedation-dentistry.html">sedation</a> services mentioned.<br />Good Luck Rob</p>

			<content:encoded><![CDATA[<p><strong>Q.</strong> Dear Dr Moore I am Writing concerning a special case of cosmetic dentistry i am interested in. I am looking for a way of reshaping 8 of my teeth (the top and bottom canines and top and bottom lateral incisors). I wanted to consult you as to the feasibility of this, and secondly if it is a service you can provide as it is reshaping the teeth rather than trying to return them to a normal shape. The effect i am after is for the canines themselves to be slightly expended (approximately 1 or 2mm) and both them and the lateral incisors to be more pointed. The intent is to be a subtle effect and must be acceptable for still eating/chewing etc. I realize in itself this request is rather unusual, but i was hoping even if you yourself cannot provide this service you might be able to point me in the direction of those who could. On that note, if you can provide this service, how much would it cost? for all 8 teeth and which would be the best way of approaching this? I would greatly appreciate any help your expertise could lend to this matter.</p>
<p><strong>A.</strong> The reshaping of teeth can be carried out in a number of ways including simple bonding of white filling to the teeth (cost &pound;50) or veneers/crowns (starting from &pound;450).
