I ll have lower front teeth extracted could you advice whether to ve dentures or bridge?

Q. I am about to have lower front pincer tooth extracted, can you please advice on what to get, a partial denture or a bridge and what type. My dentist has not explained the options, please can you help me decide I am at my wits end with worry. As soon as possible.

A. A denture is the quickest and simplest method to fill the gap.  I would recommend this for immediately after having the tooth removed, after this you could have a bridge or an implant if you want a permanently fixed in tooth.

Is there anything that can be done as I dont want to wear retainers for life?

Q. I had braces for a year, and ive took it off, and started wearin retainers, but my doctor said that i have to wear the retainers for a life now, i want to know is there anything that i can do, because i dont want to wear them for a life?

A. I recommend lifetime reatiners to ensure the teeth don’t move back.  You can have a fixed retainer placed behind the front 6 teeth and then only wear your other retainer at night.

I need the correct treatment for gummy smile

Q. Hi My problem is a gummy smile which has bothered me for some time now. I have recently just finished quite a lot of dental treatment including invisalign treatment, plus a new bridge, veneers and crowns, whilst my teeth look great I still feel I have a gummy smile, is there any additional treatment I could have to correct this? Many thanks 

A. Ideally the gums could have been lifted before your veneers were made.  It is very difficult now without having to replace your veneers.  You could have had crown lengthening treatmnet which would have reduce the gum amount on show.

A. You could look at short term orthodontics such as 6 month smiles or invisalign.  You really need a cosmetic consultation to assess your options.

What is the best treatment for gum recession?

Q. I have severe gum recession and three front teeth that are loosening,one quite severely.I really find the idea of false teeth repulsive and am really worried.Is there a solution that does not involve having dentures please.Thankyou

A. The best solution is dental implants this will give you back teeth that are permanently fixed in .  You need to get a full assessment done with a cosmetic dentist that does implants.  Your bone levels will also have to be assessed.

What is the best treatments for crowded teeth?

Q. Dear Mr. Patel Let me allow that I introduce my problem.I am 21 and my cief complaint is my upper and lower crowding. Info – Relationship of frontal teeth: Overjet: 2-3mm Overbite 2mm, Midlines upper is OK lower 1,5mm to the left, Dental crowding severe, Crossbite UR2-LR3. Next year year I’ll study at London Business School and I think that this is good opportunity to correct my teeths aswell. I want to ask you is in this case possible to correct teeths with Veneers, removable braces or Invisaglin braces? Because I realy want fast solution and I think that veneers are the best but on the other hand I am also prepared for longer treatment and wear braces but only which I mention in upper text. I just want to hear your advice because everywhere offer me just Damonbraces and similar braces which I realy do not want to wear because they do not fit my lifestyle. I am looking forward to hearing from you soon. I am also prepared to send you photos of my teeths,smile,x-ray scans if this help you to give me advice. 

A. Veneers can create a lovely smile but the downside is your teeth have to cut down, this means they will need a lifetime of treatment. As your teeth are already crooked it would be very difficult to create a nice smile with veneers without removing a lot of natural tooth. My preferred treatment would be orthodontics, invisalign is the most discreet, 6 month smiles is the quickest and damon braces are very good for very crowed mouths.

Need solution from veneer teeth grinding

Q. I have had 4 porcelain veneers done in Sept 2009 on my front teeth and wish I hadn’t now as have had problems in that after the permanent ones were placed on one came off after 3 days as it appears I grind my teeth at night. I know the veneers are slightly longer that original teeth which i didn’t agree too and so now everytime i talk, they ‘catch’ with the bottom teeth. I am concerened this catching will lead to other falling off prematurely so want them altered so they are same length as original teeth. Will there be damage to underlying teeth if these veneers are taken off to be replaced?

A. They can be replaced without causing problems but the first thing to do is go back to your dentist and see if the bite can be adjusted.  You may benefit from a nightguard that can reduce grinding.

How quick can a braces straighten the teeth?

Q. Hi Dr Riten, I just got traditional braces on my upper teeth cause of overbite my dentist said it would be removed after four weeks and then retainer will be placed in mouth which will exert furthur forced on teeth. I have approx 5 mm overbite.Have you gone through such sort of cases and is it possible to straighten the teeth in 4 weeks time? i am so confused kindly response? and also my one gum is bleeding everyday after braces place in??

A. It does seem very quick , if the teeth are not in the correct position in 4 weeks I am sure your dentist will prolong the treatment.

A. I think the tooth whitening system is Enlighten, it is the one I use.  Those fillings will need to be replaced after the whitening.

How to whiten my teeth with veneers?

Q. Hi, I have had lumineers for about 2 years,and I had bulimia for most of that time, I noticed recently that they are yellow, especially towards the gumline, I bought white strips, but it says it doesn’t work on veneers, What is the best way for me to whiten my teeth? is it possible to whiten them? please advise. Thank you.

A. If the underlying tooth is dark it is very difficult to whiten them. Whitening gels may damage your veneers the only option may be to replace them.

Could you please analyze my teeth problem

Q. Several yrs ago I had 5 front teeth worked on. 3 are on a bridge 2 were on posts. 2 yrs ago one of the teeth broke off at the root (abssesed) and had to have it pulled. Now one of the front teeth which is on a post is loose. I went to a dentist and he tried to remove cap but couldn’t. He used what he called a blue bos. Felt like a hydaulic hammer. Now he says he will have to cut the cap off, replace the post & make a bridge for the front tooth, the tooth next to it (currently have to wear mouth piece and use one of my incisors for the bridge. My concern is what if the root which has had 2 prior root conals done on it (freak, yes) also breaks off at the root. The other side of this is fron tooth capped, next tooth capped, next tooth incisor tooth missing and part of bridge. So we are talking about my entire front teeth. I hope you understand and can help. I am worried. My dentist can’t get me in until Dec. 3rd, although he got me in in a week of never seeing him before and then again that very same day.

A. It sounds like you need to have the teeth fully assessed if they are that weak you may need to think about dental implants, which will provide you with new teeth and last a lot longer.

Could you advice me on straightening of teeth?

Q. hello Doc, I am 34 years old and I have a primary tooth between my front tooth and the #2. it protrudes infront the #2, which is still there but twisted behind it. the primary tooth is also shaking slightly. I’ve been to the dentist and i was told i might have to do an extraction. the dentist also gave me this explanation for my top row i hope you can understand. 432112234 the 1s are the two front teeth and they count in both directions as you can see, i have two #2s on the right one of them being the primary tooth… my smile is not one im very proud of so is there any advice you can give me in terms of getting it straightened out? thanks a mil!

A. Its difficult to give you exact ideas without seeing you but one option might be removing the extra tooth and then having them straightened with something like invisalign.

A. You could have invisalign teen which is a clear brace, 6 month smiles is more for adults.  approx costs is £1800-5000

My dentist gave veneers and crowns as option Plz help

Q. I went to a local dentist and ask about my options for my two front teeth that are SLIGHTLY worn away on the bottom. he said he would do crowns. I said NO. BECAUSE I didn’t want so much of my teeth to be taken away. He said he would do veneers. I left the office devistated because he took my two front teeth down to miniature chicklets. He put on temps that are joined together in the center and are very tough.

I am sick over this.He is charging more for veneers than crowns. He has now choice to me but to do crowns. What can I do. I called the office and left a message to call me immediately on Monday. What is my recourse?

A. Ideally you should have discussed this before any treatment was done, sometimes cosmetic bonding can repair chipped teeth in a minimal way.  If the teeth are now prepared veneers are the better option.  Please talk to your dentist.

My front teeth hv started to ache Could this be Sinus?

Q. HELLO DR PATEL. I HAD A ROOT CANAL ON A LARGE UPPER MOLAR. IT HAS NOW STOPED HURTING. I CAN EAT PAIN FREE ON IT, TOUCH IT PAIN FREE, TAP IT PAIN FREE. HOWEVER IT FEEL WEIRD LIKE THERE A SMALL AMOUNT OF PRESSURE IN IT . MY FRONT TEETH HAVE STARTED TO ACHE SINCE THE ROOT CANAL. MY JAW ALSO ACHES NEAR MY EARS. I HAVE HAD THE BITE ADJUSTED AND ITS PERFECT. COULD THIS BE SINUS? KIND REGDS.

A. If there was infection it could take a while to settle it sounds like the symptoms are reducing so I would wait until everything is pain free.  It could be a sinus infection or the fact that the bite was too proud.  Leave everything for at least 10 days unless symptoms get worse.

How can I get rid of ulsers Plz advice

Q. recently had four inplants, prungs, on the front bottom. I am getting ulsers or sores on back side. How can I get rid of the ulsers to see if it the dentures I am wearing. You see, I had to wear my old dentures a week after the inplants. And could not take them out. The drilled holes in them to take the four prungs. I recently had new dentures. They told me try them for awhile. After the ulsers heal, we can see if the dentures work or fit. How can I get rid of these ulsers. I take them out at home so air hits the ulsers/ Rince my mouth out with warm salt water aand even rinced with peroxide… yuk Please advise. I had the dental work at aaffordable dentures.

A. You may need to get the dentures eased away from the protrusions a little more as they are probably rubbing.

A. Black tea, green tea, red wine and then coffee.  Tea without milk is the biggest stainer.  Soy sauce can also stain teeth.

Why to hv full root canal procedure when there is no infection?

Q. #14 molar had cold and sweet sensetivity. USC Dental school removed decay close to pulp, pulled nerves, filled cavities, put temp crown. told there is no sign of infection. If nerves are already pulled why go to the proceedure and cost of full root canal , Why not return for full crown only, Cavity was deep soft dentin close to the pulp i believe was the cause of pain

A. Nerve removal is root canal treatment and is often carried out over a number of visits.  If the infection has penetrated the nerve it can a while to to diagnose it completely.
