Still experiencing sensitivity after removal of wisdom tooth?

Monday, March 8th, 2010
Q. I had a surgery last week to remove my 2 wisdom teeths on left side. My dentist suggested to do another surgery to remove 2 wisdom teeth on right side next month. Before the surgery, I had ceramic fillings for 8 teeths and he polished later. From that day onwards, i could not eat any hot or cold foods or beverages. Feeling very sensitive. Am not much comfortable with it. Am using a tooth paste which is especially for sensitive teeths. My doctor informed me that, the sensitivity will go after the removal of wisdom teeth but still it continues. Please advice.,
A. Hi If you have concerns, i would express them to your dentist or seek a second opinion.

Does cost of treatment seem reasonable to you?

Monday, March 8th, 2010
Q. Dear Dr Caplin, I would be very grateful to recieve your view and informal advice. I am a British ex-pat living in Bahrain and following a visit to the dentist today, I would like to get a second view. The dental practice is well regarded but in Bahrain I am cautious of local health care. I have had some discomfort in my upper left middle molar (the one next to the back molar). FYI I have had my wisdom teeth removed some 15 years ago (I am 35). The discomfort is in the form of a sharpe pain with some cold foods, and recently sometimes when chewing. The pain does not last beyond a split second or so. The dentist x-rayed the area and did not mention any problems or decay. He said I simply needed to floss 3 times a day. He said (whcih I saw on the x-ray) that there is a gap between this tooth and the back molar of about 1mm or so, and that this is the cause. Meat gets lodged in the gap and apparantly causes decay/acid which is causing the pain. Simple flossing will stop the pain. He also recommened that I consider a bridge between the two teeth to block the gap and prevent food lodging. My first question is: Is this the only problem that is likley to be occuring? It seems a little straight forward. Is it possible that regular flossing will stop the pain and sensitivity? Or could there be something more significant going on? My concern then arose when he spotted my two front crowns. These are in need of replacement as they are over 15 years old and I have some gum disfiguration (very minor) from the few occassions I have knocked the caps out in my teens. He then suggested that if I had a budget I should get the front caps sorted to give me a better smile and use floss for the molar. My concerns are not cosmetic, but are with the pain in the molar so I want to ensure this is sorted as a priority. He quoted me GBP 1,200 for the molar bridge and GBP 1,400 for replacement of the two front caps. Having not had major dental work done in the UK for some time I would be grateful if you could advise if these costs seem reasonable. I’m sure they are not the cheapest in comparison to some UK dentists however, I would prefer to pay an appropriate amount (i.e. not the cheapest) to recieve good quality treatment. I look forward to your response at your earliest convenience. Regards Mr J Wolfe,
A. It sounds as though the appropriate tests have been carried out – the fees also seem reasonable. I would advise you not to have the bridge before your pain has subsided from flossing the area in case there are other issues with your tooth

Hi Dr Andrew I am a 28 year old female who has 6 of

Friday, March 5th, 2010
Q. Hi Dr Andrew I am a 28 year old female who has 6 of her front teeth shooting out (Overjet) with gaps in between them and my lower teeth four of them are a bit misaligned A dentist said i have flared incisors another one recomended damon braces and a third dentist recommended the six month smile. I am a bit skeptical about the six month smile (Clear fixed braces). I really do not mind any kind of braces but I am a bit confused. Could you shed more light on the 6 month smile braces cos I am confused about which one to use. Does the six month smile system really work cos i want my front and bottom teeth to be straightened and my cross and uneven bites fixed. Thank you.,
A. 6 month smile is a quick way of straightening relatively mildly crowded teeth. if you need more extensive bite changing treatment then conventional treatment might be best

Would braces be covered by the NHS?

Friday, March 5th, 2010
Q. Hi Dr Caplan, i am 27 and have what i consider really crooked teeth which are probably caused by an overlapping bight. I have arranged through my dentist to see a specialist about braces or what would need to be done and my dentist says it would probably require surgery to align the jaw. Would the NHS cover any of this? I dont have a high paying job, but have a crooked smile really affects my confidence and i really want to get this fixed.,
A. This is unlikely to be covered by the NHS

Do I need to have surgical flap for tooth removal?

Friday, March 5th, 2010
Q. I have an infection in UL4 and I have been told that the tooth needs to be extracted. My dentist says that the extraction may require a surgical flap (cutting the gum). What can I do if I would prefer a flapless extraction?,
A. Go with your dentist recommendation, he will only lift a flap if it makes taking the tooth out easier for you

Smoked since 8 and need to improve colour of teeth?

Friday, March 5th, 2010
Q. Ive smoked since I was 8 years of age im now nearly 23 my teeth are very yellow still, even after I paid £500 (which i saved long and hard for) to have them whitened privatley as im a single mum on benefits and couldnt get it done on the nhs, I undergone two weeks of sleeping with specially made trays in my mouth with the bleach, but after about 2-3 days after all the treatment, they went back to nearly the same colour. I’m at my witts end what can i do to whiten them, i’ve stopped smoking and i dont drink much tea or coffee, I’ve not laughed for over 10 years for feeling too ashamed what can I do?,
A. The colour of your teeth has been darkening over the last 15 years – 2 weeks of bleaching is not likely to be enough. However if you continue bleaching , may be up to 6 months , you will sees a great result – it is time dependant

Hi. I am in the middle of replacement veneer work

Friday, March 5th, 2010
Q. Hi. I am in the middle of replacement veneer work to my top front 3 teeth, and have been wearing temporary veneers for 4 weeks now. These came off about 3 weeks ago and as my dentist was away, another (non cosmetic) dentist in the practice cemented them back into place (cement had not been used originally). The temporary had to go back on as the prepared teeth were causing excruciating pain, even when just exposed to the air. This concerns me as I had a different experience originally (20 years ago) with minimum discomfort and no temporary veneer required. I am also greatly concerned that the dental cement will obviously need to be removed before fitting the new veneers. Could this mean that the surface of the prepared teeth for which the veneers have been made will now be altered, and will this ultimately affect the fit of the veneers? Should I ask for new impressions to be taken and new veneers made? I’m due to have the final veneers fitted this week and I would be very grateful for your opinion/advice before I go if possible. I’ve recently discovered that my dentist has deliberately misled me on two counts, and so I don’t feel able to trust that he would answer my questions truthfully, and now feel in a very vulnerable position. I appreciate that an official ’second opinion’ isn’t possible in these circumstances, but would be grateful for any information or advice you feel able to give. Thank you very much.,
A. The important fact is that you didn’t need temporisation before. If the dentist prepared the teeth the enamel may have been removed. Its a subject of debate by all agree that the margin of the veneer is best bonded to enamel not the root dentine. However there are occasions when this isnt possible.
Without a clinical assesment I cannot be specific as what action to advise.
If you have lost confidence it is usually best to change dentist.

Do you know of a cheap implant dentist in the north east?

Thursday, March 4th, 2010
Q. Can you please tell me where in the north east can i look to select a reputable dentist that is inexpensive, yet well qualified and proffessional, i have been given a quote of £2000 per implant, this is out of my price range by far, but i do want the best possible treatment available..thank you for your time,
A. Unfortunatley £2000 for implants is pretty low and you are unlikely to find quality dentistry for much less

Will braces damage caps already in place?

Thursday, March 4th, 2010
Q. I am 45. I sucked my thumb until I was 26! I also had a slight left side cross bite. I wore an upper plate brace when I was 11 but wasn’t given anything to keep the teeth in place, so over time they went back to how they were. When I was 30 I had my front teeth capped, as you could not see any of my upper teeth (and I mean any) when I smiled. My mouth was also sucken as you can image and I had deep lines. They brought two teeth over the bottom set of teeth to rectify the cross bite and lengthed the teeth so you could see them. I have a very long lip also so you cannot see my gums. Although the caps have improved my smile and support to my mouth area vastly, my upper set of teeth is still very narrow and the uncapped teeth go inwards, not giving me much support. I am considering braces, however I am frightened the caps which I can’t afford to replace will be damaged? Please note that I am told my teeth are very healthy, it is just that the upper set are displaced. I cannot tell you how trumatic my showing no teeth, while I was growing up has been for me, with people not liking me until they got to know me etc. Even the cutting down of my otherwise healthy teeth and having caps has been a huge truma for me, I’ve never really felt comfortable with the caps. Also can you recommend a good cosmetic/orthadentist. In central or west London. Thank you.,
A. If the crowns are well made they should be fine during the orthodontic treatment – I am unable to recommend specific dentists

A. You may need deeper cleaning or if the problem persists sometimes removal of the teeth and replacement may be needed instead I’m afraid as they must be quite infected.
