A. There isn’t a toothpaste that will whiten your teeth. The best thing to do is to have a whitening home kit from the dentist. There isn’t any other way to get around it unfortunately

Q. Dear doctor, I suffer from the injection procedure , this means that i do not like the pain at that moment , is there any dentist that uses the new wand system on the nhs. thanks for your time

A. I have tried the wand and it isn’t pain free. I have had it done on myself and it does hurt a little bit and it takes about 3 times as long as a normal injection. It also still involves a needle!
I would ask the dentist to use anaesthetic gel first and then apply the injection.

I have a missing upper left molar I have been recommended to have a bridge. What are my options

Q. I have a missing upper left molar due to the fact that I had acrown fitted which did not take. I have now been recommended to have a bridge fitted, but do not feel that I have had the different types explained to me. I have been quoted £450 & £600. I have been told that one of the proceedures is called Maryland but after reading the details on the website I am even more cofused, The procedure shown to fit a bridge seems to consist of two crowns with a false tootha attached would this be the higher quote do you think. What procedure would you recommend? Regards 

A. The best thing to replace the tooth is an implant. If you have a bridge a marland probably wouldn’t work. It is less destructive than a normal bridge but they have a tendency to fall off especially when replacing a back tooth because these teeth take more pressure.
A normal bridge requires the two teeth on either side to be drilled down and then the false tooth is joined to both of them. These bridges do have a finite life and will ultimately fail. They can last between 10-15 years depending on the strength of the retaining teeth.
An implant is the best option because it has the best success rate, and it doesn’t rely on the other teeth for support

Can ceramic crowns be built up? I suffer from TMJ in the worst way

Q. Can ceramic crowns be built up? I hope that’s not a dumb question. I suffer from TMJ in the worst way. My dentist put crowns on all my molars to correct my TMJ. Now it’s worse than ever. I think the top crowns are too short. i think he did this to get my bite in line. the pressure on my joint is constant now not to mention everything else. my front teeth are now banging against each other when i talk. i really believe they are too short. I’m in debt because of this dental work. I cant afford to have them taken off and redone. I’m hoping they can just be built up.

A. They cant be built up but this is will cost as much as having them replaced and unfortunately if you don’t have this problem sorted out it can result in worse problems with your tmj and eventually your teeth will start to break or it will begin to affect your gums.
There is some good news, if you have some splint therapy and your bite is equilibrated your jaw problems may subside and you may only have to have 1 or 2 replaced. Don’t go back to your dentist, it doesn’t seem like he knows what he is doing.
There are 2 options to go for, one is a Dawson trained dentist and the other is a Kois trained dentist. These are 2 schools of occlusion in the USA, and these dentists will beable to help you.
You may find this information out on dentists websites or by asking them. If you live within commuting distance of my practice I can offer advice and splint therapy, I am Kois Trained. With the replacement of crowns and addition of lumineer veneers will gums recede and create more plaque?

Q. My dentist is going to replace crowns on the 2 front teeth. He has also suggested to put lumineer vaneers on the two teeth next to the crowns, for cosmetic reasons. Will he have to scrabe the teeth first? Will there be any problems in the future ie if the gums recede? ( I am 53 years old). Will the teeth be affected in any way because of the vaneer ie collect more plaque? especially near the gums?

A. There is a possibility that you will collect more plaque by your gums above the lumineers, your dentist need to make sure he doesn’t leave a ledge and you need to make sure that you are meticulous with the cleaning. Your teeth will not have to be shaved down, if they are then your dentist is not carrying out lumineers. I am not a fan of them and they are only suitable for particular situations but if it means that the teeth don’t need to be prepared then it will be worth a go.

Q. Hi can you tell me if a crown with a post comes out can it be fixed back also if a back tooth has broken a lot becaue the crown came off can i have it crowned again

A. In both cases it all depends on the the quality of the tooth left. In the case of the tooth with the post it will be best if you have an implant. Having a new post and crown made will be money down the drain. Posts are the last thing that you can do to a tooth. Once this fails it really should be taken out. I would have to see an x-ray of the tooth where the crown has fallen off. Usually if there isn’t any decay it is possible to be crowned again

A. Hi,
thank you for your question
You could have treatment carried out on the NHS. You may be eligible for exempt treatment which means that you wouldn’t have to pay anything at all. The difficulty is finding a NHS dentist still has openings and is taking on new patients. You would have to have the treatment carried out under local anaesthetic. The dentures would probably be made first and then on your last denture appointment the teet would be extracted and the denture would be fit.
 If you jave to have the treatment done privately the procedure would be same, the dentures will be better quality and there may be a chance of having treatment carried out under sedation which means that the teeth can be extracted and you won’t remember anything.
Obviously private dentistry costs more. You could be looking at £3000 for the entire treatment

Can a veneer be applied to a crown?

thank you for your question
You can apply a veneer to a crown but is not usually a satisfactory end result. It is better to have the crown replaced as it is probably the same price and you will get a much better end result. The new crown can be matched to you bridge if you are concerned about the restorations will look

Q. Dr. Suri, I am interested in porcelain veneers for my upper and lower front teeth. Two of these teeth are crowns. Can porcelain veneers be placed over my crowns. I do not want lumineers. Thank you, 

A. thank you for your question
If you are considering having veneers placed on your upper and lower teeth I would strongly advise that you have the crowns replaced as well. It will ensure that your new restoratipns match and they will be made in the correct proportion. You will compromise the end result if you keep the 2 crowns especially if they are at the front of the mouth.
Also when cosmetic treatment is carried the bite is also taken into account and if this needs to be altered in any way to improve the longevity of the new restorations it may be necessary to replace the crowns. It is very to let you know for sure without seeing you first.
My advice would be to have a consultation with a cosmetic / restorative dentist and find out what your options are
I definately agree with you about not having lumineers,

Is it possible to have an implant bridge to replace a snapped bridge?

Q. I am in the midst of a ‘dental disaster’. I had an abscess on a tooth that was part of a bridge. My dentist referred me to have it out, and to do so they cut the bridge and took out the offending tooth. The clinic suggested I go back in 6 weeks to have an implant if I could afford it. That was 2 weeks ago. Since then, when eating breakfast on Sunday, the rest of the bridge snapped off taking the ‘fake’ tooth and snapping my remaining tooth off at gum level. I now obviously have a huge gap. This gap is upper left, I think these are 4, 5, and 6 (my left). I have been back and agreed to have 2 implants as I can’t afford 3, but now I’m researching a bit more I’m discovering that perhaps 2 implants could be enough for a replacement bridge for all 3 teeth. Do you think that is possible? I am reading that upper teeth are slightly more problematic than lower jaw teeth so does that rule it out? I would rather get it all done in one go with 2 implants if I can. I have a sedation appt next Wednesday to have the snapped tooth out and 2 implants put in. I had impessions taken, but I’m assuming it’s for the temporary plate – but would that be for the implants – in other words, is it too late to ‘insist’ that I have a 2 implant bridge? Please advise. Thank you very much.

A. I am sorry for the delay in answering your question and you may have already had the sedation. If you ahven’t had the implants p[laced then you can definately ask to have an implant bridge. This is a viable treatment option as long as there is enpough bone. This will depend on how much bone was present around the teeth and how carefully the teeth have been removed.
If you have had the implants placed next to each other it is very possible to still have a bridge. The crowns on the implants would be joined together and the back tooth could be attached to these.
I have restored many cases this way and haven’t ever had any failures.
 Your dentist needs to make sure that the bite is correct once the implants have been placed

Is braces a good solution for underbite?

Q. My Daughter is 12 years old with an underbite, her dentist recommends braces, what would be type would be the better solution?

A. Probably a combination of functional appliances and fixed braces. It will depend on the cause of the over bite. If it partly due to skeletal shape then the functional applianceas are worn to correct this. The lower jaw is postured forward with the brace. After this the fixed braces are used to move the teeth back into an ideal position.

Q. What are the risks or complications associated with dental bridges?

A. bridges involve drilling down the teeth on either side of the gap. If the teeth are heavily restores they may end up needing root canal treatment. Also the 2 teeth on either side are supporting the weight of an extra tooth which puts a strain on them.
Bridges last approximately 10 – 15 years. They usually fail because one of the supporting teeth begins to decay as the cement holding the bridge on begins to crack. At this point it may not be possible for the affected tooth to retain a new bridge. This means that the bridge then has to be made longer or implants can be placed.
If you can afford to have an implant now that would be the best option

Q. i have been diagnosed with significant bone loss in my front teeth.My dentist wants to remove my 4 front teeth and supply dentures.As two of the teeth are not loose would a crown be able to be fitted and a bridge on the two loose teeth.He says bone loss is there in the two other teeth but they are fine at the moment and i would rather not have them removed

A. If I was you I would not have a bridge placed on 2 loose teeth. If the 2 front teeth are the ones that need to be removed then the side teeth are the the 2 weakest teeth in the mouth and cannot support a 4 unit bridge. Whatever you have done you need to make sure that it lasts and that it is the best treatment plan for you in the long run. The most successful and long lasting bridge that can be placed in the mouth is a bridge from canine to canine. A bridge can be placed on these two teeth even if they are slightly loose. It may be better in the long run to sacrifice all four front teeth and have a 6 unit bridge placed from canine to canine. If you are not keen on this idea then there is the option of implants. You will need to check that you have enough bone left for this. If you have bone loss make sure that that you don’t have gum disease. If you do and it is not resolved then everything that is placed in your mouth will ultimately fail much quicker than it should. Your dentist should check that your gums are healthy before definitive treatment is carried out.

Will the crown have to be removed and the root canals cleared?

Q. I have a problem with one of my lower right side teeth. It had a root filing around 1990 with no problems. About 5 years ago the dead tooth stated to break up and so about 2 years ago it was fitted with a crown again with no immediate problems. However about a year ago I stated to get small abscess in the outside gum just below the crown line. It would occasionally bust releasing a small amount of milky fluid and then heal but only to reoccur a few week later. Now the discharge is more puss and erupts every 4 to 5 days. I can feel the pressure of the build up increasing in the interim. I presume I have an infection in one of the root canals? Will the crown have to be removed and the root canals cleared? Many thanks for your advice.

A.

The best thing to do is to ask your dentist to refer you to an endodontist. They will be able to tell you the prognosis of a re-root canal treatment. If it is possible the best thing to do may be to have a new crown placed because if the existing crown is drilled it will become weaker. If you don’t want to disturb the crown it will be possible to have an apicectomy carried out. This is a small surgical procedure where the infection is removed directly from underneathe the tooth.

A.

If your bridge has moved you may have extra pressure on the bridge and this means that it will eventually fail. You need to see a dentist that has a knowledge of occlusion, this is how the teeth function together and the way way they fit together. This is crucial because repacing the bridge needs to be well planned out. If you don’t want to have dentures and your bridge doesn’t last, you could always have implants

A. Full mouth implants can cost between £25000 and £50000.
It is very expensive. You can have implant retained dentures, these are much cheaper. You would need to go to a dentist to have a consultation to find out whether you are suitable for the treatment

After removal of my wisdom tooth, my teeth are spacing out

Q. Before I had my wisdom teeth removed I had beautiful straight teeth. But in the past four years since the surgery some of my front teeth have spaced out. It bothers me because I always used to get compliments on my teeth. What can be done besides braces or veneers to close these gaps and s it expensive?

A.

Removal of wisdom teeth doesn’t normally cause movement of the front teeth. This is something that can happen any way. If your teeth are splaying and have spaces check with your dentist to see if you have gum disease. If your gums are fine then the spaces can be closed with white filling materials. These are not expensive so it could be worth trying and if you are not happy with the results then you can consider braces o veneers
