Is it possible to change the post that are in the gums?

Q. Dear Dr. Moore, I have four crowns (in the front in the top rig)that are now showing a black line and so need replacing with Porcelain ones, but also my gum is quite dark and not pink. Is it possible to change the posts that are in the gum, or is it possible to lighten the gum in any way? Thank you for your help! Regards

A. Hello the gum can’t really be changed but the posts might be taken out and replaced with White posts and porcelain crowns

Q. Hi, just wanna ask. My 2 molar teeth was missing is it possible to feel the gap on my teeth? also, how much is the cost of it? Thank you.

A. The costs depend on what is put in the gap. a denture is the cheapest option bridgework the next  and implants the most expensive. expect to pay between £3500-4000 for 2 teeth held with implants

Is root canal required before having a crown in my case

Q. i asked my doctor to give me a nice smile or correct spaced teeth in the front of my mouth and he is giving me a crown and root canal on the damaged tooth first, but what about the other teeth in the front they are still spaced will i need a root canal on the other teeth in order to get them unspaced and looking nice?

A. It is difficult for me to say without seeing you, all I would say is that you don’t always need root canal before  crown or veneer is fitted

I need general anaesthesia for my dental treatment but can’t find one

Q. dear dr moore i am hopeing to have some implants but i have a extreme fear of dentists so i have been trying to find one who will give a general anesthesia i have found two clinics which say they do and wene you phone them they talk you out of it and say they wont do it and its there in black and white. so could you help me or do you know of any one. hpoe you dont mind me asking you this.

A. Hello, It will be very dificult to find someone to do this for you but you could try Ashok Sethi in Harley Street as he has access to Hospital services at a nearby medical centre. You could also try sedation which is cheaper safer and more readliy available

Half my front tooth is broken and NHS is the only option

Q. I have recently had one of my front teeth break off half way down, and I basically have a mouth full of fillings including a root canal job. My confidence has been knocked so much and I really don’t know what can be done to help me under the NHS? I’m really hoping you can advise me as it’s getting me really down.

A. You can get basic treatment on the NHS although cosmetic treatment may need to be paid separatly for the front tooth.

Can’t make up my mind to have a porcelain or a metal crown

Q. I’ve had cosmetic crowns placed on my 6 upper front teeth (eye tooth to eye tooth). There were multiple issues and I am having them redone. The issue that I’m faced with now is whether or not to have the back of the crown porcelain or metal. If I go with porcelain, I may need another root canal (they will have to remove more of my tooth to make the back thinner). If I go with the metal, I’m afraid that it will show when I laugh or yawn and that later it may look dark. What would you advise?

A. It is unusual to see the back of upper front crowns and metal would be stronger, if you are very worried about the apperance you may have to risk needing a root canal under the porcelain crown

I need to quit smoking to have implants so is bonding an option?

Q. I am aged seventy two and I have had upper crowns and bridges for about twenty five years, now two of the teeth are crumbling inter palatally. This involves the left central incisor and the canine tooth. The remedy which has been suggested is to cut of my bridge between the central incisors, have the teeth removed on the upper left and then have implants. Until I get a new bridge I will need to wear dentures, the estimated cost for this is £9k. There will be two dentists involved in this; one says that I must stop smoking and the other dentist say that I do not need to. At present I am hoping that “cosmetic bonding” is an alternative. My questions are. Could “cosmetic bonding” prevent the offending teeth from additional crumbling? If implants are the only solution, will I need to stop smoking? Many thanks.

A. Bonding is a good temporary solution although if the teeth need to come out then it might not work that well. If you have implants stopping smoking will help improve the success rate.

I need to straighten my tooth is my age a concern?

Can I have implants if I have bone loss?

Q. I have bone loss, non smoker all my life and no medical conditions. I am awaiting 3 top front teeth extrations (have been waiting 2 years), one hanging on by a small thread! hopefully I will be measured for a small plate. Taking daily pain relief and limited in what I can eat and very embarassing talking to patients myself as, I have a fractured tooth and it has dropped. Am I able to have implants if I have bone loss? was offered by dental hospital to have teeth splinted, but was told not a pretty sight, what do you think? Kind Regards

A. It sounds like implants might be the best option although you’ll need x-rays and an assessment to decide if you’ll a suitable candidate. Let me know if you’d like me to take a look.

Narrow teeth have caused lot of spaces between the tooth

I have very narrow teeth at the bottom and am 63 years old the gaps have become more pronounced over time I have been adviced to put veneers on bottom teeth is not easy is this the case.I have had them widened slightly built up and laser light at the tips they look a bit better is there any other option please

A. I guess if veneers aren’t possible then you might have to try a brace to close the spaces.

How good is white filling to have tooth whitened

Q. Hi i have had root canal in one of my front teeth and it has become slightly grey,my dentist told me the only way to whiten it is to bleach it internally but theres no way i can afford it.is it possible to have a white filling in the front tooth instead and would this have a whitening effect?

A. The white filling would let the grey colour shine through so it doesn’t work too well

Glueing a replacement tooth to adjoining tooth an option?

Q. I have one missing front tooth & currently a single tooth denture that I am not happy with. I have been told I can have a replacement tooth glued to the adjoining tooth instead. I am told this is a successful procedure although not often used. What do you think?

A. It might work in the short term but an implant might be a better long term solution

Unfortunately teeth have moved forward after braces

Q. Hi, I used to wear braces before but this was a while back now. After my braces were romved i was given retainers which i only wore for a few months… But i now feel as if my teeth have moved forward as they used to be before and i really hate it! Is there anyway i can get my teeth slightly back? Hope you can help me.

A. You might have to consider having the brcaes back on i’m afraid or maybe straighten then using porelain veneers

Tongue thrust is causing my front teeth to separate

Q. Hi There, I have a tongue thrust, and it is causing my two front teeth to separate in an upside down V. This is concerning to me because I wore braces for 4 years (about 7 years ago) and did speech therapy to re-train my tongue. I have a night guard that I wear although not regularly because it is uncomfortable, despite that it is in rest that the pushing occurs. Would veneers be a viable option to correct this cosmetically, while continuing to wear a guard at night?

A. Yes veneers might help but you’ll need a new reatiner to stop them moving

Wisdom teeth is coming through and need a braces is it covered under NHS?

Q. Hello there, Sorry to bother you but I have two quick questions if you could answer them please? I have previously had fixed braces (both) when I was younger, my bottom brace only stayed on for 4 weeks instead of 6 months as planned. I then had them both removed (after having top on for 1 1/5yrs prior to bottom) and wore the retainer 24 hrs a day for the year. I then went for the yearly check up point and my retainer was taken off of me. My teeth went wonky within months, and I have been too scared to go back. I get really upset about my teeth and suffered alot with braces to still have wonky teeth. I am 19 and in full time education and have swapped dentist now. My wisdom teeth have been coming through for a year and causing me grief, will they fit braces with the wisdom teeth growing through and are they likely to get funded by the NHS? Sorry its so long and hope its understandable for you to answer. Many thanks

A. Hi, thanks for you questions. I wouldn’t expect you to get braces on the NHS and you may need to have the wisdom teeth removed before you have any other orthodontic treatment.

I am depressed about my teeth condition

Q. Hi After years of neglect ,all my top teeth have badly decayed ,and broken off . Im so embarrassed about this ,and hence cant face a dentist because of the shame that I let my teeth get into this state. Unfortunately I cant afford private ,so will have to go nhs , I would like to know what is available ,as I know I am probably going to have to have them all taken out and have dentures,but I have been told nhs wont cover this? Also ,what would you advise me to tell the dentist before my appointment ,as I want to let him know the state my mouth is in before he actually sees them ,im so depressed about this and really need some help .

A. You will be able to get denture treatment on the NHS. I would be worried about what the dentist might say, thats our job to fix peoples teeth and we are very used to seeing less than perfect mouths!

My second root canal has gone horribly wrong

Q. I had a root canal done on a mandibular 2nd molar 5 days ago, was already being treated with antibiotic as a precaution. After the novacaine wore off I was in a great deal of pain. Took vicodin and antibiotic as prescribed, called the dentist the following morning. He switched my antibiotic to a stonger one and said he would see me next week. I was awake all night tat night again in pain, called his emergency number, now accompanied by swelling an an inability to eat due to pain. He returned my call suggesting that I make an appt after the wknd was over to come in to have the tooth extracted. I am 44 years old, female, in good health. This is my second root canal and experienced nothing like this with my last one. Even the exterior of my face is painful when touched. Any suggestions on what has gone wrong or what I should do?

A. Sometimes an infection is so severe it won’t clear wth root canal. In these cases sometimes removal of the tooth is the only option. You could also ask for a referral to a specialist in root canal , an endondontist.
