Should I get my decayed molar teeth extracted now

Q. I am due to have my upper left molar extracted next month at the dental hospital under IV sedation. I was referred there by my dentist due to the tooth being too badly decayed underneath the filling to have root canal treatment on it. As it is causing me little or no pain at the moment I would like your advice as to whether I could wait until it does cause me pain and have it extracted then or would this just make things worse? Also, I am concerned about the gap it would leave behind and would like to know my options regarding this, as neither my dentist nor the dental hospital have discussed this with me. Thank You

A. Given that the tooth decay is bad, it will be getting worse over time. It is unlikely that you will be able to arrange, at short notice, extraction with sedation when the tooth starts to hurt. You need to have the tooth out at the time that is convenient to you not when you get toothache ( eg on your holiday etc )

Gaps can be filled with dentures, bridges & implants if the adjacent teeth are healthy. All need regular care & maintenance.

Q. Dear Dr Glaiser Six years ago I wore braces to separate out two front teeth that were overlapping. This worked (up to a point) but now you can fly a Zeppelin between my lower and upper teeth. Is there any way this could be corrected? Thank you

My teeth seem to soft No matter what I do how can i get it fixed

Q. I dont know where to start. im 33 and my teeth are in very bad repair. A lot if the work carried out over the years has failed. My teeth seem to soft, No matter what I do, Brush 3 times a day, mauth wash, floss, nothing seems to help. I have developed a major fear of the dentist now, to the extent that when in the chair my chest tightens and have been known to cry. My 2 lovely boys 5 and 2 have to be taken to the dentist by my wife as I just cant face it. I know I have to go but I dont know how I can, Im 33 and my teeth of a 80 year, I dont smile, my gums are always sore. With the current was the world is, Im not sure I can afford anything. Just if I could weake up one morning and my teeth have been replaced I would be the happiest person alive. Its got to that point in my life that I dont want to laugh or even talk with people because of my teeth and the feeling of detures, well I guess thats all I have left to look forward to. Sorry going on to much but just had enough. thanks 

A. Contact your local Primary healthcare care trust (PCT ) Dental Contracts and ask if there are any NHS Dentists with IV sedation contracts. Then go along for an assesment which will cost £16.50 to find out if they can help you.

How much would you charge per tooth if the patient wanted 18 teeth done?

Q. If a patient wants all his/her teeth veneered (10 teeth on top and 8 below), do you have a cap on the price you charge (e.g a total smile makeover for X amount)? If not, how much would you charge per tooth if the patient wanted 18 teeth done? Thanks for your help.

A.

I comply a bespoke treatment plan for each and every one of my patients and the costs involved depend on the patients natural teeth, what they would like to have done and the time it is going to take.

The initial appointment is £150 for a full examination and x-rays. From here a treatment plan is constructed which outlines all appointments and costs involved to achieve what the patient is looking for.

How can i find out that the bridges on my front teeth are zenos or not?

Q. Dear Andrew, i had 2 x 3 unit (top front 6) bridges replaced as they were 20 yrs old, i had them replaced with Zeno Zirconium which look & feel fine. I also a 4unit bridge fitted to my lower right side & 2 crowns to my left upper & lower, all Zeno Zirconium. I had all this done 10 weeks ago & am worried now that they (other than the 6 fromt) don’t actually look like the Zeno’s i have in the front…. I had all the work done in Budapest. What is the best way for me to find out if all the ones other than the front are Zenos or not?? I would really appreciate any correspondance from you, Thanking you.

My mother is diabetic is she able to have implants in all her mouth?

Q. Hi there i have a question my mother who is diabetic, taking tablets. she has all her teeth moving and seem like they can fal out at any time. dr said to her that the bone in her mouth is finish which actuarelly hold the teeth. and i want to know is there anything we can do for her, is she able to have implants in all her mouth, she also experience sevear pain when she eating drinking or anything. plz do right back to me as i will be waiting. thank you very much for your help and time

A. It sounds to me like your mother has chronic periodontal disease and in this case she would need to see a periodontist to see what can be done to save the teeth she has left.

If your dentist has told you that you mothers bone is shrinking then this may be a problem if you are thinking of restoring the gaps with implants as implants are fused to the bone. Although there are things you can do such as bone grafts to deal with this.

Last week I got my teeth removed and dentures fitted but still the pain exists advice?

Q. Dear dr Glaister , I`m really hoping you can help me. I recently had my teeth removed ,and dentures fitted (straight away).I`m petrified of dentists ,and they were made aware of this…hence i was supposed to get both local injections ,and councious sedative? NEITHER worked ,and i sobbed while they yanked and pulled at my teeth..I can recall everything. I also don`t think they put in the right dentures ,as these are not the white ,nice ones i chose ,and was shown by my own dentist. My real problem now ,is after a week ,these throb ,ache ,and i`m bleeding from my gums/mouth still…..also i`m not happy with my so=called new smile?? And could i possibly have an infection?The entire process took less than 9/10 mins ,and i`m still in alot of pain. ANy help would be gratefully appreciated..many thanks

A. Dear,

I am sorry that you have had such a negative experience at the dentist.

Any aesthetic issues you have with the denture can be corrected easily by having the denture remade. This time ensure that you and your dentist are agreed on the shade of the denture to prevent further issues. However the first thing to be addressed should be the pain. It may be that you have an infection and/or that the denture is causing irritation. Either way you will need to return to the dentist to have these problems dealt with. Unfortunately I cannot diagnose the problem without assessing your mouth.

Would i be better looking at implants for my teeth?

Q. I am in the process of getting a bridge with 8 teeth fitted this was suggested due to the fact I previously had a bridge fitted to replace a canine tooth some years back. I recently had to have the other canine removed as I had an ulcer under this. This was treated a number of times over a few years but it continued to return so it was suggested this should be removed. It was then suggested that I have a full bridge covering 8 teeth all of the top front and the 2 at the other side of the removed teeth as this would look much better. My question is would I be better looking at implants? My present dentist says this is the best way forward for me stating implants my not look as good. Thanks in advance for your reply.

A. A bridge will normally mean drilling down teeth to anchor it in place. when a bridge fails it normally takes another tooth with it so the whole thing needs replacement. i would definitley look into implants as this would keep the teeth  independent from each other and avoid the need to drill down teeth. perhaps a 2nd opinion might be worthwhile?

Would be possible to just get a cap or venner on just that one tooth

Q. I really only have one tooth that i have a problem with, whenever i smile, it would appear my teeth were crooked but its only because i have a chipped and slightly crooked front tooth. the chip isnt very big, but it is noticablee by far. and the crookedness might not look so severe if the chip werent there. So what i was wondering is, instead of getting braces, because there no where near in my budget, if it would be possible to just get a cap or venner on just that one tooth. Out of my two front teeth, its the one on the rightt, and its crooked towards the left, witch is where the chip is, on the left corner on my tooth.

A. If the chip is as small as you are discribing then it can be fixed using a technique called bonding. This is doen by hand by a Cosmetic Dentist and simply builds the tooth up using different coloured resins matching the exact colour of your tooth.

My front teeth is starting to erode away can u advice on the options

Q. Hi, I have lost 7 teeth which have rotted away all back teeth, one of my front teeth is starting to erode away aswel it is getting really embarrasing now as i know my breath smell, my gums often bleed and it is making me really depressed, i cant afford to get them done is there anywhere that would be able to help me, i would find it really embarrasing going to the dentist, any suggestions would be grately appreciated.

A.

I would strongly recommend that you go and see a dentist to sort this out for you. Please do not feel embarassed as we are used to seeing dental problems and are here to help you.
Once your teeth have already begun to decay, only a dental professional can help you with this by treating or extracting the teeth in question. However, you can do something to make sure the teeth that are still ok do not go down the same route and this would be to make sure to are brushing and flossing twice a day and that you are keeping food and drinks with a high concentration of sugar in them, such as sweets and coke, to a minimum.
The smell you are talking about is from the decay in the teeth and it will not go awat until it is removed by a dentist so I would ask around friends and family for a dentist they recommend and book an examination with them.

Ive a chronic snoring problem is oral maxillo facial surgery the only option?

Q. I have a chronic snoring problem and a sleep consultant is referring me to an oral maxillo facial surgeon as I have a receding chin and a large overbite. He feels that these factors are causing my snoring problem and sleeplessness(I do not have sleep apnea}. At 53 is surgery the only option, if so what would that involve? Also, could this problem be causing breathing problems when I exercise? I am a keen road cyclist and walker, but have huge problems with ‘coughing’ on exertion. I am using steroid inhalers, but without much success.

A. A recessive chin can cause a restriction in the airway and negatively affect airway function.

Will a cosmetic dentist reduce the length of the crowns fitted?

Q. I have recently had six crowns fitted on the NHS,which are a combination of metal fused to porcelain. My crowns are too long and ‘cut’ into my bottom lip.I asked the restorative dentist at the time to reduce the length,but he refused. I have been left feeling very self-concious. Is it possible if i went privately,to a cosmetic dentist,to reduce the length or will this crack or split the crown?

A. This may crack or split the crown but this is unlikely if water coolant is used.  I often recontour osrcelain but would be wary as these crowns were made on the NHS and  would not be sure of the support of the porcelain by the underlying metal

Would braces make the gum recession and TMJ worse?

Q. Hi, I am 27 and am considering braces since my bottom teeth and quite wonky. Although my teeth and gum themselves are healthy, for most of my life have had a clicking jaw joint. I have been using a mouthguard but it has done little to help me stop clenching my teeth. I also have sensitive teeth and some gum recession cause either by toothbrush abrasion or maybe the clenching of my teeth.I am looking for a dentist who may be able to help me with all these issues! Would I be able to have braces or would it make the gum recession and TMJ worse? How much a metal cap would cost and what are my options

Q. Hello, When I was younger I fell over and chipped my front tooth in half. It was capped. After a few years it fell out and I went for more dentistry work. The dentist pretty much ground the tooth down to a little stump and then bonded a cap to the tooth. He said it will last a few years and so far has been in for a good 8 years with no signs of movement. I am unhappy with the tooth as it is rather grey in comparison to the other teeth… my friend has a cap on the same tooth and his is greyish also. Unfortunately I smoke but am planning on quitting in the near future. I have made my mind up on getting the cap replaced and was wondering on the options available. I would love a fancy cap… if not solid silver at least the outer edge silver and the rest white matching the rest of my teeth. I wore braces as a kid and my teeth are straight. When I went to mexico I noticed a lot of the guys had the gold outer rimmed caps and thought they look rather nice as they aren’t as much as a solid metal tooth. Could you please give me a rough estimation on how much a metal cap would cost and the options available to me. I am after it having a silver outer edge if that makes any sense? Could you please advice on my sensitive and decayed teeth
