Q. I have good, straight teeth that are very dark as a result of antibiotics for chest infections when I was a child. What quality of result might I expect from chemical/laser whitening?
A.
In my opinion, the best whitening treatment on the market at present is a deep whitening system called enlighten. I hav had a few patients who have had improved colour with tetracycline stained teeth. I need to stress though, that it is not guaranteed and it all depends on how continuous you are with the home whitening.

Q. Hi, 2 years ago i got a thin surface put on my front teeth and slightly lengthend as they were very rough and serated at the bottom. The coating has now gone discoloured compared with my own teeth – will any whitening product remove the stains? i get married next month and would really like me front two teeth to be the same colour as my others thanks
A. Tooth whitening has no effect on fillings you need to have them replaced.

Q. Hi im 16 years old and i have a gap in my tooth, the gap is more wide at the bottom of my tooth than it is at the front. Looking at my photos i have not had a gap in my tooth when i was younger but as i reached the age of about 12, i started to get a gap in my tooth. I have been looking at “veneers” on this website and i was wondering whether i can get them at this age or will my gap get better as i get older or is it better if i get braces, however i do not want to get braces unless i have to because mostly my teeth is straight, it is just the two front teeth. Can you please recommend to me which method i should take to get rid of my gap? Thanks
A. This is quite a tricky one. Braces will be perfect for you to close that gap at the front. This will be the safest and least invasive form of treatment. There are options available on the market that work quickly and are more discreet than they used to be. I need to stress that you will need some form of retainer after the teeth have been moved as they may drift back again otherwise.
Alternatively, veneers can be made to give the illusion of the teeth meeting. The only concern that I have is that it may make your 2 front teeth appear too wide. It may be an option for you to consider having 4 teeth veneered instaed to make it look more even. At you age though, braces is the best option- as having veneers will mean ‘preparing’ the teeth to fit the veneers on. This means drilling healthy teeth. Hope this makes sense and good luck. Discuss things with your family and see what they think.

Q. Hiya Dr Patel, I’ve been looking into various treatments to close the gap between my 2 front teeth. The gap is approximately 2-3mm. Whilst researching treatments such as invisalign and inman aligners i have came across a product called Teeth Effect Bands, these claim to close the gap without the need for visiting a dentist. They apparantly act like an elastic band that draws your teeth together. I was wondering if you fell these are an effective treatment and would you recommend them for my issue. Thanking you kindly
A. I have heard of these I am very suspicious of self treating your teeth you need a dental professional to help you. I would recommend fixed braces or invisalign.

Q. I am 26 years of age and have been fitted with a crown which i paid £750.00 on my front right tooth as after root canal treatment the tooth had darkened. I believe that the crown has a life of around 10 years. What are my options after this, can another crown be fitted if this one comes away? Or is an implant the only option?
A. A new crown CAN be fitted at a later stage if the tooth structure is strong enough. Root canal treatment tends to weaken the tooth itself, but I would be tempted to play it by ear and to monitor it for yourself.
A dental implant is another good option to replace a tooth- but I would look at all your options when you really need to. I would expect that the crown that you have recently had fitted has got some years of life left in it!

Q. I have a large space at the side of my two front teeth left side, I know I am not suitable for inman aligner, nor sure about invisalign, my two front teeth are very sightly spaced but one sits out a little bit further than the other. I feel like I am running out of options now. Don’t really want train track braces and as my teeth are really healthy veneers were my last option if I emailed you some pictures would you given me your recommendation. Many thanks.
A. You are more than welcome to send me some pictures, however I have closed spaces with Invisalign but my best option would be metal braces, you could even have a combination of treatments, short term braces say 6 months followed by veneers. I look forward to seeing the pictures.

Q. My mother is 53 yrs old and she has removable dentures for last 3 years.They are becoming loose and also causing problems while eating. She has come to UK to visit us and she will be here for 2 months. Could you please let me know if she can have dental implants and how much does it cost and how long will the treatment last. We live in Rhyl, north wales. Thank you
A. It is impossible for me to say if your mother can have dental implants without seeing her in person. She will need to have a CT scan to confirm the bone levels and case planning will need to be done first. It all depends on the amount of bone present. If there is not enough bone, then implants will not be possible.
Ideally, you need to see a cosmetic dentist for a consultation to see what options are available.

Q. hi i am 33 years old and my teeth are broken and rotting i am a nhs patient but am aware they dont cover cosmetic work i am willing to pay any extra there may be but my budget is 3000 pounds i have had quotes of aroud 8000 pounds which is way to expensive i have lost all my confidence and would be grateful if you could help me find somewhere that could do what they can on the nhs and charge me for the rest thank you
A. I think you will need to speak to various NHS dentists to see what they can do for you. This sort of arrangement can only be made on an individual basis with a dentist.

Q. 2-3 years ago, my last molar on the left side had a big cavity and reached the nerve. my dentist advised me to do a root canal but i decided not to and asked her to do a filling instead. i did a composite one, and another time when it wore down, and switched to amalgam the third time when it had to be replaced (just this year). now, part of my amalgam filling has chipped off. i am not sure how big the chip is in comparison to the cavity but i don’t feel any pain. i think my dentist will want to replace it again but i am concerned about the negative effects of drilling away the amalgam (it releases harmful mercury vapour). what should i do? should i still get the filling replaced? can the chipped part just be filled up with another filling?
A.
You need to ideally discuss your concerns with your dentist. The filling can be re-done or patched up. This is entirely your call. To patch it up, you can expect it to chip off again- and having it re-done may be the best option- but still not guaranteed to NOT chip again. Make sense? If you have concerns regarding the mercury vapour, then discuss this with the dentist. If you have a new amalagma filling, then not much of the amalgam has to be removed in order for more to be added. In order to do a composite filling, then ALL the amalgam has to be removed. It can also be patched up with a materila called glass ionomer that will repair the filling. If the tooth is heavily filled, then it is always advised to have a crown for protection. Hope this has helped.

Q. ihad my teeth shaved today in prep for 6 new veneers im horrified seeing them shved down and have to wait a week for my veneers to be fitted; will i be happy with the result? i hope so seeing my teeth like this in their raw state i am now terrified.
A. you need to remember that having veneers effectively means prepping your teeth to house the veneer.
Some practices work alot differently. I would always advise the dentist to construct a diagnostic wax up of your ideal smile- so that you can effectivley see the end result before your treatment starts.
i’m sure your veneers will look lovely. If you are ot happy with them- for whatever reason, you nee to speak up and say so before they are fitted. that way ant adjustments can be made in the porcelain beforehand.
Good luck, and be sure to stress your concerns with your dentist.

Q. Hi, I have made some poor judgements. I had some veneers fitted that I keep having problems with. 3 veneers have fallen out and have been replaced, one is now chipped. I did not have them insured, can I insure them now? If so, who would you recommend?
A. I think you may find it difficult to insure existing dental work but it certainly is worth a try – I would suggest trying to call Denplan and see if they will cover you

Q. Hi I had 6 veneers and 2 crowns fitted about 3 weeks ago but am not happy with the colour (my dentist says they are B1) My temporaries seemed to be whiter. My dentist says i can have them redone but this would obviously mean going through the process again and he says they look natural but looking at them i know i am not totally happy and wish the were whiter. I wanted to know how hard or easy it is to have them replaced and would you advise i get them redone.
A. If you are not entirely happy with them, then I would strongly advise you to have them re-done. If you aren’t happy with them now- chances are, you won’t be happy with them 2 months down the line- or even in a few years. You need to come away and be 100% happy. B1 is the lightest shade on the natural tooth shade guide. Any whiter and you will be into bleaching shades- as long as you are willing to keep ontop of the whitening for your existing natural teeth, then you shouldn’t have a problem. Hope this has helped and you sort it all out

A. It really depends on how much bone you have left in your jaw, you could ave 3 implants and secure 5 teeth off them. prices vary approx £1500-2000/implant and £500-1200 / new tooth.

A. I’m not sure what you mean when you say a silver tooth? Some practices offer silver/metal crowns. Amalgam fillings are also used which are silver in colour. I need to stress though, that these restorations are only used when absoloutely necessary. If you think you need dental treatment, then your local or regular dentist can advise you further.

Q. Hi, 17 yrs ago I received a root canal, a post was inserted and a temporary crown was attached to my left front tooth. When replacing my crown I was told that apparently the post needed to be removed and replaced as it was 17 yrs old. But after removing the post it was evident that my tooth was cracked all the way up, so the dentist removed the tooth and built a resin cantilever bridge using my other front tooth as the abutment. As the colour of my tooth could not be matched a veneer was placed over the abutment. There were no impressions made of my teeth before this occurred, the ‘fake’ tooth was simply built. This worries me as I keep reading that impressions need to be made prior and a technician will make the permanent tooth. Is this true? if so can the veneer be removed from the abutment so that an impression can be made?
A.
It is very difficult to give advice in this situation, because i am not completely understanding the circumstances. If it is a case of removing a veneer and replacing it, then this should be possible.

Q. My daughter has seen an orthodontist for braces and we were told her bottom jaw is growing faster than her upper jaw. The braces are not recommended until surgery is proform after she stops growing to pull back her bottom jaw. She is 12 now. I would like to know if there are other options besides surgery.
A. In these cases to obtain an ideal final result surgery is often the only option. perhaps your Orthodontist would discuss other options in more detail although with these she might not end up with the best possible result

Q. Dear sir, I am a 27 year old medical graduate.I have spaces between all of my upper and lower incisors and canines which have gradually developed over years.Also, I have receding gums in almost 8 of my upper and lower teeth. So, around the roots, the teeth look very yellow. All this makes my smile look pathetic and I feel so less confident while smiling.Please suggest me with appropriate solution for my teeth gaps and receding gums. Best regards
A. Firstly, I would recommend that you see a dentist/cosmetic dentist to see how your smile can be improved. It is difficult for me to advise you as I haven’t quite seen your teeth for muself.
Receeding gums are usually caused by poor oral hygiene. PLaque and calculus can build up underneath the gum if flossing and brushing isn’t adequate. this causes the gums to become red and infllamed and eventually receed. This can cause the teeth to eventually become mobile a sthe bone is affected.
You CAN stabilise your gums with regualr dental and hygine visits and mainitaining your oral cleaning routine at home. Receedign gums can also make your smile appear more gappy. I would defiately advise you to stabilise your gums before considering further dnbetal treatment. Crowns/veneers can make your overall smile appear more aesthetically pleasing, but please attend a consultation with your dentist first to discuss your issues.

Q. I lost a top tooth, fourth from front, due to infection four years ago. My dentist fitted a bridge to the next ajoining tooth which lasted three years and then decayed underneath. It kept falling out and in June my dentist removed it, used the remainder of the tooth,strengthened with metal pins and bridged from the my next tooth at the back. On July 6th I went to have my permanent bridge filled. The temporary had been fine but sensitive to cold, which I mentioned. The temp had to be drilled off and this was very uncomfortable in contact with cold, which I mentioned. When the new bridge was fitted it felt fine apart from sensitivity which I hoped would improve. It did not and became worse with my jaw and ear aching and so I returned to the dentist last week who gave me 400g ibuprofen and anti -biotics as he said there is now an infection in the root. He implied that is was my fault as I had not returned early enough, however I had explained about the sensitivity when it was being fitted and was told it would settle down. It had only been fitted two weeks before. I paid £1200 for this work and am very concerned about the resolution of this problem and that I may loose the tooth and bridge. I am also due to go on holiday in less than two weeks time and have lost faith in my dentist. Please advise.
A. It might be possible to either root treat the tooth through the bridge or remove the bridge, root treat the tooth and cement the bridge back in. Either way it sounds like you need to get this sorted ASAP. You could try another dentist maybe an Endodontist who specialises in root canal work if you have lost confidence in your existing dentist? Q. hi Dr Solanki, I am 34. My two front teeth protude slightly and seem to go to one side, i think this may be related to my tongue pushing against them when i sleep and when my mouth is relaxed. They seem to be getting worse as time goes on. I was thinking about the six month smile as i’m not keen on having braces for 12months or more. Can braces really work in 6months, it seems a short time for something that normally takes well over 12months?
A. The 6 mont smsile treatment normally takes APPROX 6 months- it all depends on how severe your crowding is. It is a newly developed orthodontic technique that moves teeth quickly,effectively and safely.
My nurse is currently wearing the 6 month smile brace and she has noticed a difference in the first couple of weeks.
