Q. Hello, I’ve been speaking to my dentist about having my teeth whitened. He advised me to go with the take home kit because he believed, the 1hr laser was much more risky in terms of causing damage to gums and blood supply to the teeth as well as an increased risk of tooth sensitivity for up to 6 months. He also said that there was no difference in effectiveness between the 2 techniques. Would you agree with both these statements? Many thanks!
A. The take home kits, in my opinion are much less convenient- but can often have a more desirable colour lift over the days. There may be sensitivity from both methods, which is common, but nothing that can’t be resolved from sensodyne toothpaste!
I recommend ENLIGHTEN as an excellent whitening system.

Q. I had orthodontic and later orthognathic treatment in my 20s to fix an open bite. During the orthodontic treatment I suffered irritation and subsequently an infection in the gum around the 1st molar of the upper right side of my mouth, which was not helped by the bracket surrounding it. The area was left untreated for some time (one year approx) and after treatment, involving root planing from my dentist, resulted in a large area of gum recession and some bone loss. Thankfully, when my braces were removed the gum swelling and bleeding gums eventually disappeared I have developed recurring deep pain in the affected area that extends to my upper jaw as well as sensitivity where the root is exposed and where a gap appeared behind the tooth. I’ve approached my dentist about having a gum graft to cover this unsightly area but have been knocked back and told that nothing can be done about it. I feel like I’m being fobbed off and given the minimum consideration especially as my teeth and gums are in otherwise good condition. Do you know of a good periodontist in the London area who could help me as this is making me very depressed? I’m located in West London. (By the way I’m now 33yrs old)

Q. I need to have an implant fitted. I am also told I have a couple of loose teeth from bone loss on my lower jaw. Again it looks like I will need implants but I am on benefits and dont no how I am supposed to afford this. Please can you advise me what to do. I lay awake night after night worrying about this. I keep regular appointments with my dentist and have an appointment with a higenist to try to stop the problem getting any worse but my dentist said he cant do anything about the ones that are already loose. Please please help.
A. Regular hygiene visits are paramount in stabilising your condition. Implants are an expensive- yet long term treatment option. Many dental practices offer interest free payment plans to help. I can understand that it is difficult to finance the treatment. Alternatively, you can always opt for different treatment. Dentures are a way of closing gaps where teeth are missing, but this is not a fixed option. there are always bridges that can be done.
Before you start on any treatment, you need to have a full mouth assessment. To have implants placed, you need sufficient levels of bone to place the implant. It sounds as though you have generalised bone loss anyway. Implants may not be the best option for you, but without seeing you in person it is difficult for me to comment.

What age do I have to be to have veneers- and is there an age limit?
A.
We generally advise people to be over 18, but all cases are different. There is no age limit, as long as your teeth and gums are in good condition. You will need a full oral health assessment first with xrays. Not everyone is suitable, but you need to have a consultation with your dentist to discuss the options that are right for you.

A. If the teeth are not completely decayed or broken down you may be able to have root canal treatment and crowns placed on them. Ask your dentist to see if he/ she has any other options.

Q. Dear Sir, i stay in india age about 26. I do have a very big problem since my childhood that i cant smile freely due to the gaps between my four front teeth. I want to go to dentist. Sir, I want to know how much time it will take to close the gaps.

It all depends on what type of orthodontic appliance you need, if it is removeable or fixed braces- or if you decide to have veneers to close the gaps. Also, prices vary from dentist to dentist- I’m unsure of the prices in India, but I would advise that yousee a cosmetic dentist for a consulttaion to discuss your needs and options.

Q. I need two crowns on the top front teeth. I have been told the difference in price beween private and nhs options but my dentist would not say either way which is the best option can anyone tell me what i will be getting for the difference in price.
A. As a rule a private crown should be made by a better quality Lab using superior materials. You should also have more of a choice regarding the type of material used and the overall look of final result

Q.
what is the best manual brushing technique?
A. These points are good practice when brushing your teeth:
1. Hold your brush at a 45 degree angle. The brush should touch the gumline and the tooth
2. brush in small circular motions- concentrating on one tooth at a time
3. Repeat the same process on the inner surface of the teeth
4. tilt the brush vertically behing the front teeth- uppers and lowers
5. Brush the biting surface of the teeth aswell.
6. Use a medium toothbrush- one that isn’t so hard as it may cause damage to the gums
7. use a brush that isn’t too big- as the bristles need to reach in lots of small spaces.
Your hygienist will teach you this if you are unsure- and always remeber to floss or use inter-dental brushes.

Q. I have a gap on my top fron incisors which is slowly expanding (at 2mm at the the moment). one of them is also rotated clockwise by 5 degrees. How can this be fixed as economically as possible?
A. You might need to see a dentist for some quotes regarding orthodontic treatment, you might also consider using white filling to bond to the teeth to make the gap look smaller

Q. At what age can you have veneers fitted ?, my daughter is 15 and the two front teeth are quite prominent, but doesnt want a brace or anything like that
A.
At the age of 15, me along with the majority of other dentists will recommend orthodontics. We usually say that veneers is an option for the over 18’s- when you are old enough to make the decision yourself as an adult, but all cases are different. As her teeth are not disformed or discoloured, but the position is wrong- then ortho is the first line of treatment. There ARE alternatives to the standard fixed braces- such as invisalign. Tese are invisable braces. I would advise a consultation at a cosmetic practice to discuss her options.

Q. Where teeth need to be extracted prior to having implants, does the gum need time to heal prior to placing implants or can the proceedure be carried out straight away?
A. If possible and in the right situation i would place implants as the teeth are removed. If there are infections or extensive bone loss then waiting for 4-6 months for bone healing might be advisable

Q. I maintain a very good oral hygeine but i usually have bad breath just after hours of brushing. I have used some of the expensive brands in the market but it still persist. Please give me a helping hand. Cheers
A. Bad breath is a condition called ‘halitosis’
It is often caused by poor oral hygiene, strong foods such as garlic and spices, drinking caffeine and alcohol can cause you to become dehydrated- reducing the saliva in your mouth- you need saliva to rinse the oral cavity from bacteria.
Also, smoking is a big factor.
You need to stress your concerns with your dentist and have regular hygiene visits. Hope this helps and thankyou for your enquiry.

depending on the exact situation you could have a brace cost between 1500 to 3250 depending on severity
or veneers if you want something speedy cost 475 to 575

Q. My son is just over a year old. One of his front top teeth looks as if it is craked or like it’s two teeth growing in instead of just one. Is this anything I should get looked at or is it too early? It doesn’t seem painful so it must not be craked I imagine.
A. This is nothing to be worried about. It sounds as though the tooth and the tooth next to it could be fused- almost like siamese twins. The other possibility could be that when the tooth was developing, the bud splits on formation into 2 teeth.
When teeth are fused, the nerve can be susceptible to nerve damage and can sometimes develop cysts- that are often painless.
The second set of teeth may not necessarily be fused- but if you are unsure, don’t hesitate to ask your dentist. He/she may be able to take xrays, but it is not anything to be concerned about.

Q. I have very serve lupus and have had major problems with my teeth and gums… When I was younger I had a brace fitted to my top and bottom teeth twice but this only flared my gums making them red and prone to bleeding.. It was not known that I had lupus at this time so they were removed by my dentist.. I have horrible un slightly teeth that are uneven and cause me a lot of heartache and self confidence issues. I now have a gap where I have lost a tooth. I was told that due to my medical condition that I may be eligible for cosmetic dentistry on the nhs, if this is so would you know how I would go about looking into this and it’s becoming more of an issue for me. Thanks
A. You will probably need referring to a teaching hospital where they deal with complex mediacl dental issues. Ask your dentist if this is possible for him to do

Q. Hello. I had a fixed brace fitted for two and a half years when i was 14 to straighten my teeth. The main problem at the time was over crowding causing a descending canine tooth to protrude over the other teeth. This was sorted out by the brace and my teeth were perfect. I am now 36 and the canine tooth has with time slowly began to move outwards so it is becoming prominent and is spoiling the curve of my teeth. Would i have to go through the whole process again (bar removing any more teeth as i had 4 taken out last time) or would it be a simpler and cheaper procedure as all the work has been done previously, such as just having a retainer of sorts? A. Hi- I would advise that you see a cosmetic dentist to go through your options. there are more discreet forms of appliances on the market- from things like invisalign(a removeable clear appliance) to 6 month smile- a fixed, clear brace. After the treatment has finished, then you will need to wear some sort of retainer to stop the tooth from moving back again. This can be in the form of like a mouthgurad that you can wear at night time. Ask your dentist about the options and prices. Good luck with your treatment.

Q. I have an 11 year old son who has “goofy teeth”. He has not yet got all his adult teeth through, would he be able to have the 6 month braces treatment, or will he have to wait until all his adult teeth are through?
A. You need to wait until his adult teeth are through- but if you have any questions- then consult with your dentist. It is highly likely that he will be referred to an orthodontist anyway if his teeth are protruding. Good luck
