Q. hi im 24 years old and have really bad teeth my front teeth are chipped and back teeth have holes in i think i will need dentures but am worried about having to keep them out over night and worried incase they fall out as its really embarrising having false teeth at such a young age can i get mini implants on the nhs so that my dentures will be secure or is there anything else i could get done.thankyou
A. I’m sorry but there won’t be any possibility of implants on the NHS, well fitting dentures might be your only solution

Q. Dr Solanki, I have a dilemma I hope you cna help me resolve. Im 29, have good upper back teeth apart from one (3rd from back) that I used to have a mildly annoying discomfort from when running. WIth hindsight I would have left it! but my dentist proceeded to root canal it and I now have a huge black filling after a few attempts to resolve. A little piece at the back recently chipped off and has been refilled. My dentist is suggesting an ceramic inlay or a crown. Am i correct in thinking an inlay wouldnt offer any benefits to a normal filling other than aesthetic? Secondly, would you suggest fitting a crown now (destroying a lot of tooth) or leaving it until it really needs it if it cracks etc? Many thanks in advance
A. An inlay/onlay are good for aesthetic reasons- but also offer strength from the material used. For overall protection- if the tooth is heavily filled, perhaps a crown may be the best option.
It’s hard for me to comment as I haven’t had chance to asess the tooth, but you ideally need to discuss all of these concerns with your dentist before going ahead with any treatment. You need to make sure that you are 100% confident and sure about it.

Q. Hi, When i last went to the dentist (i was around 11 years old) they told me i needed teeth taking out, more fillings and probably braces to straighten out my teeth and i was too scared to go back. I did also get fillings done at the time and this took several numbing injections and i still felt a strong pain and i developed a serious phobia of the drill. Im 21 now and i would really like to get my teeth sorted. But i am really scared to go back and i dont have alot of money so i am putting it off. But now one of my back teeth (next to a wisdom tooth that is squashed up next to it) is decayed and the black in it is gradually taking over the whole tooth. I am unsure whether its too late to save the tooth or not but if i need any teeth taking out / any work with the drill i dont think i can go through it. Do you have any advice on what a patient with a phobia like me could do? I dont mind travelling to get too a good dentist, my main concerns are the costs and the pain/drill that i strongly fear. Many thanks for your advice in advance.
A. Firstly, the first step is to find a dental practice and dentist who you feel comfortable with. If you feel relaxed in your surroundings, then half of the battle is won. There are also options such as sedation that you can have to make you more at ease when having treatment.
As for the cost of treatment, it is difficult for me to comment as I haven’t seen you in person for an assessment, but I can imagine that the wisdom teeth will more than likely need to come out. This is very common- and straight forward in the right hands!
I would be more than happy to see you myself for a free consultation at my kissdental practice to discuss what can be done to restore your mouth- and most of all, your smile!
As for the cost of your treatment, It would really all depend on how much treatment you will be needing. Hope it all helps.

Q. I would like dental implants to fill in gaps on my upper jaw that are currently filled with partial dentures. There is one gap on each side of the mouth. On the partial denture there are two teeth together. However, the dentist I saw about implants said I needed three each side. I would like to get a second opinion but the consultation cost each time is putting me off. Should I be able to be guided by the amount of teeth on the partial denture? Thanks

Q. i have problems with my gums will this stop me being able to have veneers?
A. Whichever dentist you go to…he/she will tell you that your gums need to be stabilised before having veneers and crowns. Ideally, you need to see a dentist for a consultation and most likely have a thorough hygiene visit. If crowns/veneers are fitted to a mouth with unstable gums, the chance of poorly fitting restorations is highly likely. Plus, you must be extra vigilant with your cleaning with veneers anyway.

Q. i have one tooth near the front that is very grey due to an old filling. my dentist says the tooth is healthy and leave it alone until it needs a new filling or crown (white next time)but i am aware that it is grey. could i have it painted white?

Assuming you are prepared to pay privately to improve the appearance I would think that replacement of the filling offers the best long term solution.

Q. Dear Mr Solanki, I would really appreciate it if you could awnser a couple of questions for me. When i was younger i had a very poor dentist who made poor work of my teeth. Since those days i haven’t revisited a dentist. Nowadays my teeth are terrible – i have a huge hole in my back tooth, another hole in a different back tooth which is revealing a filling out the side, and most of my teeth are very crooked. I have always brushed atleast once a day but it just seems like my teeth are decaying one by one. I’m 20 now and this is the only thing ruining my self confidence to talk to people. I am writing this to ask how much it would roughly cost to fix these problems – i think basically i need almost everything redone. I work full time in a small job, and as we know life in London isn’t cheap but i will save up for the next five years if you think my teeth are rescuable. Obviously you cannot see my teeth so you couldn’t say exactly how much but an estimate would be greatly appreciated. Thankyou very much for your time,
A. The very first thing that you need to understand- is that decay is caused by Sugar and bacteria on the teeth.
To prevent further decay happening, then you must restrict sugar to mealtimes and brush at least TWICE daily- with a fluoride toothpaste. Snacking on sugary food and drink between meals will cause decay.
If this issue is combatted- then your mouth can be restored. If you have a smile makeover whilst being a high risk decay patient, then the resoration work may fail, and the amount of money you have spent will have been wasted.
You’re quite right when you say I cannot give a full estimate as I would need to do a full oral health assessment, but if you are looking at a full mouth case, then expext to pay anything around £5,000 to£20,000.
Ideally, you need a personalised plan to suit your individual needs

Q. I had a post under the dental implants section of the forum entitled “too late for an implant?” I was wondering if you could advise on the following…. Since my last post I had my implant fitted in August of last year. I also had my natural teeth whitened. Last week I had my temporary crowns removed and replaced with my permanent crowns. I am due to go back for a check up on them next week. Whilst the final results are better looking aesthetically than my previous bridge on the NHS, I have since noticed that my permanent front crowns appear to protrude more than the natural surrounding teeth. They also appear slightly bigger. I suspect that this was a necessary evil in order to compensate for the gum recession I’ve experienced and an attempt to give the illusion of symmetry all round. Before I had the implant fitted, I enquired as to whether I would require any kind of gum/bone grafting as I could not understand how it would ever look the same again without this. I was advised that this would not be necessary and that he could achieve a good result without this so I went along with his recommendation. Now that I see the final result, I feel a little disapointed. Is it worth bringing this up when I go back next week for my check up? Would there be a reason why they would have not wanted to opt for a graft from the onset? Does this mean I can’t get one now in the future? I look forward to any advice you have.
A. In most situations once the implants are in place you have to work with the position that they are. Bone grafts would not alter the appearance. The graft does add another stage that is one more stage for variation to occur. They dont always work, but in the case of a block bone graft, when they dont the main downside is the extra discomfort of the procedure and the site specific risks of surgery.
It is worth bringing up because it will give your dentist the chance to respond.

Q. Hi, I need three small fillings inbetween a couple of my front teeth. The dentist has quoted £120 each which I though was a lot judging by other prices on the internet. Do you think this is expensive and orth getting a second opinion?
A.
The price is within normal fees in general practice, but is at the mid-higher range.
Second opinion do have great value, although in this case the saving per filling may be cancelled by assesment fees & nuisance factor.

Q. I am 28, I had braces in my early teens to straighten my teeth and with the looks to have surgery to realign my jaws, my bottom jaw is to big and bits across to the left and my top jaw too small. I didnt go ahead with this. I now work ina job role that is customer facing – my teeth are alot straighter but def not like when i had the braces removed plus I have 2 unsightly canine teeth 3rd in on each side. My jaw really affects my breathing and is extremely prominent and my teeth just make me never what to smile. This has also been an issue of bullying as I grew up. I now want to get this sorted but really dont know how to go about it – was wondering if you know anyone who would be interested in using me as an example case as i am aware treatment can be very costly – I can forward pictures etc for ref. thank you.
A. It sounds like you will need Orthognathic surgery that might qualify for some NHS treatment. Perhaps you should see your dentist and ask for a referral to your local oral-maxillo facial surgeon

Q. Hello Doctor
I have really bad teeth which is seriously affecting my confidence and I’m finally ready to do something about it, having been reassured by some of the advice given by you & other experts here. I am slightly worried about a couple of things, though. Firstly, is it possible to just go into the clinic and ask for him to recommend me something? Or do I have to state precisely what I want? Secondly, do I have to have all veneers fitted at once? Or can I have a couple done at each time (to spread the cost)? Thank you sooo much for all your help & I eagerly anticipate your response.
A. Hi there, Many people who come to see me are not always sure about what they need. I often find out the patient’s concerns and talk through how I can improve the overall appearance of their smile and restore function in the mouth. The cosmetic dentist that you choose to see, will recommend what they can do to improve any areas of concern. You don’t necessarily have to research anything before hand- as most treatments will be explained and discussed in your consultation.
Many practices also offer different payment methods and ways of finance the treatment- so you can pay monthly etc. Hope this has helped.

I am 9 and worried that i need teeth taken out to make them balanced
A. It is not uncommon for some teeth to be taken out to make room for the others if there is not much space to start with.
It is nothing to be worried about- as the dentist will make the area ‘numb’ so that you won’t feel any pain.
It is best to talk through it with a family member and the dentist. Hope this has helped

Q. Hi I’m 25 and need braces, but I’m single and don’t want them getting in my way when talking to other people. Could you also tell me whether I would have to pay for the 1st meeting and would I be able to pay in installments rather than a lump sum for the whole procedure? A. you need to have a consultation with an orthodontist to discuss the treatment options available for you. The price of the first consultation varies from practice to pratice. Some are free, others may charge around £100. There is no harm in shopping around first though until you feel comfortable in the right place.
Some clinics also offer finance payment options, but this should be discussed at your consultation.

Q. hello.. i would like to get my teeth straightened but i dont know what is the best procedure. I would like a good dentist who i can trust will fix my teeth the way i want them. i have small spacing on all my upper teeth but not on my lower teeth.. i have a minor overbite on my top teeth but it isnt overly noticable. i wondered about invisilign as im 24 and work with the general public and dont want everyone to know im getting my teeth straightened. I heard it will cost about £2000.. is this correct. please contact me back as it decreases my confidance severly. wanting a beautiful smile means so much to me as i am a very bubbly person, but its holding me back. thankyou so much,

In this case you would need a full clinical examination with some costs for Xrays Photos Casts etc. Invisalighn or Clearstep systems might be suitable. the costs depend on how much need to be done and by what devices. Cllear positioners cost £2-4000 approx,
