i have gappy teeth can you help

Q. I need braces for cosmetic dentistry for gappy teeth Q. Hiya. I’m 23 years of age and have never worn braces – the dentist stated the only reason I would need them is for cosmetic reasons. I’m very very self-concious of my teeth but next year I am getting married and know my teeth will put a slight dampner on my day given the number of photographs that will be taken. Eek! The main reason being they are ‘gappy’. There is a gap of about 3mm between my two central incisors and about 2-3mm gap between my central incisors and lateral incisors and again the same between my lateral incisors and canine teeth (I’m presuming they’re the right names, I’ve just looked on the internet). My lateral incisors and canines are also slightly twisted with a bit of a sharp edge. I do have an overbite, and when I asked my dentist about a quick fix previously he said that it wouldn’t look great as I would have a ‘horse teeth’ mouth – they’d look too big, and wouldn’t last very long and would cost a lot. I had a check up a few days ago and was meant to ask my current dentist about what I could do however he speaks very little English and I don’t feel confident he would understand what I was asking and vice versa. I desperately don’t have the money to do this but would probably contemplate borrowing money. I’ve trawled the internet but I’m still none the clearer on where to go, or how to start the ball rolling! I’m living in West Yorkshire if you would have any recommendations?? As it it cosmetic improvement you are looking at NHS dentistry will not apply.
If you have 12 months before the wedding and you may have some orthodontic options. if your teeth are a good shape (once aligned) and colour this may be an option. Guide price £3-6000 for typical cases and expect to have to wear a “retainer” for many years / life to keep the result stable.
Option 2, veneers lumineers, thineers with or without drilling the existing teeth to reduce the bits that stick out of line. guide price 400-800 per tooth and at least the top 6 teeth in most cases. £200 on photosimulation / study casts / trial wax up and a detailed plan before you do anything is a good idea. The benefit of ths treatment is carried out in two visits
Option 3 a short term fix, aesthetic bonding with tooth coloured filling to reshape teeth, can be done in a single visit. Guide price 150-250 per tooth.
Some practices like ours offer an interest free / low interest  loan on treatment costs over £2000.

iam currently going through the process of having two dental implants

Q. Dr, I am a implant patient currently going through the process of having 2 implants (one either side of my front teeth). I had the implant screws inserted in January and have just recently returned to my dentist for a review to find that one of the implants has failed to knit with the bone and has subsequently been removed. Although my dentist was dissapointed with this he did say that it was highly likely we could rectify the problem by inserting a 12mm screw as apposed to 8mm thus connecting with stronger bone that hasn’t had teeth in it previous. I am very confident in his ability but would just like a second opinion as to weather you think I will be second time lucky on this ocassion, and if not what my options are? Thank you in anticipation.

A. Hi

Implants generally have a high success rate in certain conditions. Unfortunately it is impossible to say if it would be successful in your case without carrying out a detailed medical and clinical examination.

If you have concerns then I would either recommend a converstation with your dentist or visit another implant dentist for a second opinion.

I have had a bad experience with porcelain veneers

Q. Hi Dr I hope you can give me some advice? I have had a bad experience with porcelain veneers and I am now on my second set of veneers which unfortunately are not right. I would dearly love to get this solved once and for all as my confidence has been shattered by the problems Ive had with my teeth. My question is, can veneers be replaced numerous times or would this be likely to put the health of my teeth at serious risk? I am in my twenties and if I were to embark on corrective treatment this would be my third set. The teeth were minimally prepared. Thank you, your opinion woula be appreciated

A. Hi
the question pertains to how heavily prepared the teeth were and how quickly re prepped as the best advice is to wait a year for the nerve to recover. if cosmetics was an issue you are advised to go into a set of diagnostic wax ups till you are happy with the result.
if speech is an issue this can take upto a year to resolve.

iam interested in getting some cosmetic dentistry

Q. Hey, i am interested on getting some cosmetic dentistry work done in the coming months – Basically i am unhappy with the length on my central and lateral incisors, i have always been of this opinion, i had braces when i was younger, and am very happy with the position of the teeth. They are just too big in a length way!I am now 24 and want to get it sorted out, is there any way i can have them filed down maybe 2mm, i don’t think i need veneers, they seem expensive. I think it would greatly help my self esteem. If i can get this done what do i inquire about to a cosmetic dentist? and would you have a vague idea of how much this procedure would cost? Thanks so much.

A. Unfortunatley you can’t just file the teeth down, except very small amounts,  as this often makes the teeth very sensitive. I would try to find someone local to you for a proper assessment and a detailed quote before embarking on any further treatment.

I have Trigeminal Neurology. pain developed after crown was replaced do i need to see another dentist?

Q. I had a 30+ year gold crown replaced 3 years ago and have had pain ever since. The dentist referred me to a Neurologist that I have been seeing for MS and it was determined that I have Trigeminal Neurology and have been prescribed medicines to control the pain in the cheek, gums, lips, etc. But I still have pain around the crown, cannot get juice, fruit, etc near it. If I bite something on the crown it feels like something is under it and feels like I am biting on tin foil. I have been back to my dentist many times and am always told the crown is fine and to just keep taking my medicine. Is it time to see a new dentist for another opinion? I have a problem of insurance and who I can see. Thank you.

If you feel you need a second opinion then I wouldprobably suggest that this is a good idea.  You obviously are beginning to loose confidence in your dentist – who may well be right but another set of eyes may cast new light on your situation

Q. I HAVE FOUR CROWNS ON MY FRONT TEETH. THEY HAVE BEEN KILLING ME FOR A YEAR. I HAVE BEEN TO SEVERAL DENTISTS AND THEY SAY XRAYS ARE FINE. THEY SEEM TOO BIG FOR MY MOUTH AND THE PRESSURE IS TREMENDOUS. WHAT CAN I USE TO HELP THE PAIN. I HAVE CALLED MY DENTAL DELTA PLAN AND GET NO HELP.

A. It sounds to me that one or more may have either an infection, or there is a problem with the bite.  Maybe you should see another dentist for a 2nd opinion

My son’s two front teeth are large

Q. Hi I have a son whose theth are just growing. The two front teet are rather laerge and he leaves his mouth open a lot. I would like to know if the fact that he leaves his mouth open alot would make his front teeth grow larger.

A. It should not have any effect.  How old is your child?  When the two front teeth first appear they can appear large as they are adult teeth and your son has not finished growing yet.  Your dentist will monitor him.

Hello, i am enquiring about silver crowns at the moment i have 2 white gold teeth on the front i have had them for about 7 years but have just realised in my reliogion its not permissible for men to have white gold in any shape or form is it possible for me to get these taken out and get 2 silver crowns on only reason asking my freind told me you cant have siver crowns appreciate your reply. A. Hi many thanks for your enquiry, most dental metals do contain some amounts of gold. the best thing would be to discuss your requirements with your dentist, it maybe possible to have platinium teeth made as apposed to silver.

Which would you recommend bridges or implant

Q. Dear Dr Caplan My 12 year old son has beautiful, straight teeth with no fillings. He’s missing his 2s (on both sides) his 3s have come through in their place. His ‘baby’ 3s are where the adult 3s should be and the orthodontist said the roots are long so he could have them well into his 20s. The orthodontist consultant has insisted leaving them all where they are and just filing the sharp ends off the adult 3s to make them look more like 2s, then using bridges or dentures if and when the baby 3s fall out. We’ve recently moved house, and our old dentist told us that the best final result would be to move the adult 3s where they were meant to be (using braces) with dentures maintaining the space the 2s would have occupied, then bridges or implants when he’s older. Which would you recommend? I’m concerned that my son will have to live with the outcome for the next 60 years and just want to make sure that the course of treatment we choose is the right one. The orthodontist was an NHS one with a long waiting list and a budget, I don’t trust that my son’s end result was his priority – but equally, I’m not a dentist and don’t know if I’m just being neurotic! Whilst I appreciate you can’t make a diagnosis, any advice would be very much appreciated. Kind regards,

The 2 treatment options that you have outlined are the usual ones stated for this type of problem.  In my view the best tretment would be to remove the baby teeth and move the canines into their correct position.  This can be difficlut to do due to the size of the canine roots.  An assessment by an orthodontist would be necessary prior to removal of the baby teeth.  The benefit of moveing the canines are the  correct placement of this important guiding tooth; the correct gum and underlying bone shape; the correct width of teeth in this very aesthetic area.

How can i correct tooth discoloration and front teeth are translucent are these about to crack?

Q. Hi, I have had a root canal done about 13 years ago on one of my front teeth. Now, I am starting to see discoloration, and the tooth has a very little chip (which can’t really be seen). My question is do you have any suggestions what I can do to fix this problem of discoloration? Crown? Venners? Also, the rest of my front teeth are very thin on the bottom, also translucent. Do you what that may be from, and how can i avoid having problems with those teeth in the future? I just think that those teeth may be more prone to cracking? Is that true? Thank you very much for answering my questions.

A. Hello

The discoloured root treated tooth could be bleached from the inside to improve its colour or covered with a veneer.
The translucency could be due to tooth wear or erosion. Chat to your dentist and see what he thinks

Can cosmetic dentistry be covered by insurance?

Q. Hi was wondring im 18 have a severe phobia of dentists due to something that happend when i was younger which has lead to an extremely bad mouth and teeth i have made up my mind i want cosmetic denistry done i have 2 questions – can u get cosmetic dentistry done through insurance if i took it out and how much on average to just go to sleep and wake up with the perfect smile if i had all teeth done/replaced hope i can get some answers would be a big help as ave really lost all confidence i have cant go out with friends things like that thanks :)

A. Cosmetic dental treatment is not normally covered by insurance unless the treatment was needed anyway to correct decay etc.
You might need a few visits of sedation, the costs vary depending on the treatment

Are 4 mini implants enough to hold a bottom denture

Q. Hi are 4 mini implants enough to hold a bottom denture in place, as dont they move at the back? And do you use sedation to have this done for nervous patients ? And is the price expensive?

A. Hi many thanks for your enquiry, the mini implants are great to stabilise dentures. the more implants that are placed the more stable the dentures but the dentures will still move to a certain degree. it also depends on how much bone is left in your jaw to decide how many implants can be placed. this treatment can be done under sedation. the cost per implant is between £400 – £700 dependant on where you get the treatment done.

Q. Hi , can you help me with an enquiry, l have just broken my bridge, last time this was replaced paying £500 towards the cost on the NHS. My dentist now says l will not be able to have it done on the NHS due to cut backs. He has quoted me a figure of £2,700 to £3,200. For any normal working class family this is an impossible amount for me to find, is there anywhere l may be able to get this done at a lower price. l live in Sussex Thank you in advance for your time

A. My best advice would be for you to contact your local health authority to see if you can find an NHS dentist willing to take on your problem

Teeth straightening questions

Q. Hi there, I have a question about teeth straightening. I had braces as a teenager but over the years my teeth have started to move causing my front two teeth to overlap. I’m now 23 and would like to get my teeth straightened using train track type braces. I am also interested in having clear braces (not like invisalign). Any information about how long treatment may take and cost would be really helpful. Thankyou.

A. Treatment time varies on what needs to be done could be 6 months to 3 years.  Costs depend on who you see and what type of treatment you have.

A. Hi many thanks for your enquiry, the best option to sort this problem out would be to have the teeth veneered. this would correct the misplacment but also the pointiness of the teeth. the average cost of this treatment would range from £1500 – £2000 dependant on where you had it done.
