Q. I am a 32yr old woman who has batteled normal teeth problems like cavities all my life but nothing to major aside from 1 root canal.I recently had teeth 20 and 28 extracted…and have a feeling that 2*which had a root canal) and 14 will have to be removed. Im interested in replacing the missing teeth especially after reading how the surronding teeth can shift over time. I am unsure as to which type of replacement to get….any ideas?
A. Implants are good for missing teeth especially if you don’t want to drill down the adjacent teeth for a bridge. Implants also stimulate the bone reducing later shrinkage. they can also be placed at the same time as the tooth is removed. Tooth 28 is your wisdom tooth so replacement is probably unnecessary

Q. I have been having troble with my tooth for some time and mu NHS dentist has advised a crown. Could you please advise me how much a crown with a titanium peg would cost(well the range of charges) ? The tooth is, well not the front two but the second one back on my right hand side. Also how do I find a dentist to undertake the work ? we live in Carlisle. Many thanks
A. IF you mean to take the tooth root out and replace with an implant its abut £2000- 2500 from start to finish. If the existing root is still solid a post and crown is around £500-800. Both private fees . If you live in Carlisle, University towns like Newcastle have plenty of implantologists the Association on dental implantology (www.adi.org.uk) will carry members details

Q. I have had TMJ for many years, and the last three years I have suffered with a displaced disc which meant I had chronic facial pain and the disc used to become inflamed quite regularly. My dentist recently gave me a splint to wear 24/7 for about 8 weeks which helped with the facial pain but did nothing for the disc. I then started to experience terrible headaches with the splint so the last time I saw my detist he said he could do no more for me and sent me to a specialist in London. This specialist would like to perform open surgery to re-attach the disc and then adjust my bite. Another specialist has said I could have an arthroscopy. Is it possible you could tell me which would be best please? I was hoping the disc might go back without surgery but it looks like it might not be the case. Many thanks

I do a lot of work with patients suffering with the symptoms you have. I also beleive that surgery is a last resort.
At my clinic the type of TMJ treatment offered is via Neuromuscular dentistry. I would recommed coming in for a consultation to see if this method of treatment can help you before considering any surgery.

Q. my teeth are in really bad condition there ones broke and had one removed i haven’t been to a dentist in a long time as im scared but i have a little girl who i want to look after her teeth so means i need to plus my confidence is getting worse for reason to do with my teeth i would like to know if its possible to get my teeth sorted and how much will it cost
A. You need to find a dentist that can spend time with you and offer you all of the treatment options. Without looking at your mouth I cannot tell you what the cost would be. It depends on the condition of your teeth and your treatment choices

Q. Dear Dr Kailesh The injections given in your mouth to make it numb before drilling etc. do not work on me. They numb my lip, tongue, gum etc. but NOT the nerves in my teeth. I have been treated by dentists using sedation which at least helps me get through it, but I do still remember some pain. Are there any gum injections that are more likely to numb the nerves in my teeth so I don’t need sedation as my dentist has told me he is going to stop doing it and I’m desperate! Thanks.
A. hi many thanks for your enquiry, there is a stronger anaesthetic called articaine which can help improve the effects of anaesthetic. my advice is see if this helps.

Q. Dear Sir, I have recently had a porcelain bridge fitted at the front (upper) as I lost my front tooth to an abscess.I am happy with it cosmetically but as my top teeth protrude the dentist sanded down two porcelain teeth at the back as to avoid them catching the lower teeth. Originally he said they would be remade as I paid privately and he said they would be covered by insurance; but now says he has permanently cemented them. The two porcelain teeth that have been sanded are rough on my tongue and I wonder can they still be adjusted.
A. A simple poslish with porcelain poilishers wil usually remove the roughness to produce a glass like finish.

Q. hello i am currently living in weymouth and most of my teeth have snapped and i have 8 missing one in front and rest on lower and upper parts of mouth can these be fixed with a doudle set of dentures and if so wot price would it be and will it hurt as i am scared of dentists hence the teeth missing plus i need to get them fixed asap as i keep getting swollen mouth and gums quite alot many thanks
A. NHS fee is £198 for treatment including extractions and dentures, also factor in the need to replace the initial dentures within 6-12 months due to shrinkage / jaw changing shape after healing. Private fees are variable. NHS fees have some exemptions contact NHS direct for details .However it sounds like you need to get it fixed ASAP

Q. I was thinking of having the 6 month ortho and i was wondering if you knew a place where i could have this done. If I go to leicester which is where you are located it would take 7 hours on the train as i live in gloucestershire. Also are there any requirements, I basically have one tooth crown on the right which is slightly in front of the other about 1-2mm, the tooth next to it is slightly back about 3-4mm. So in effect I need the right crown pushed back and the tooth next to it pushed forward. I asked a few years ago if veneers were possible however they said no, at the time though i was on NHS, can pay for it now, no problem with money issues anymore. Hate the thought of braces unless they are invisible, would want the quickest method possible. Any suggestions on a location, any other help would be great. Thanks

It is very diificult to give you a treatment plan without looking at the mouth. The options you have to straighten the teeth are
1. conventional orthodontics
2. six months smile
3. invisalign or clearstep.

Q. I have a scratch on my front tooth, that has become really quite visible. It now appears like a small chip out of the front of my tooth. It has recently become sensitive on eating or drinking anything. I am extremely paranoid about this as the tooth appears to be very weak and i am extremely concerened about its appearance. What kind of treatments could cover this scratch/chip, and how much would these be likely to cost.
A. Rather than a scratch it sounds like a crack it needs to be assessed as teeth with cracks can breakdown. It may just need a simple filling but if the crack is very deep it may need a crown.

Q. My 10 year old fell and chipped off the wing bites of his front two teeth. The dentist has repaired them using bonding but he is worried when eating and won’t bite with them because he heard the dentist advising me about being careful when eating hard food. Is Lumineers an option for children? Thank you.

im afraid i do not advise veneers for children except in extreme cases as there is an increased chance of root canal being needed

Q. How can one floss an already plaqued teeth?
A. Fisrt see the Hygienist to remove the palque and tartar from the teeth. This will claen all the debris from your teeth to make all subsequent oral hygiene measures more effective

Q. I have a sudden outbreak of gum disease next to my only implant which was put in over 2 years ago and has never given me any trouble. I am careful about brushing, flossing and cleaning so this is unexplained. Your site mentions “other drugs can even cause abnormal development of gum disease“. Could you tell me what these might be? Could creatine have any effect? Thank you
A. Hi
Firtsly it is very important to see a hygienist every 3 or 6 months for a 20- 30 mins scale around the mouth, especially if you have implants. Bacteria can get under the gum and cause and inflammation to occur , if it is not regularly cleaned.
i would stress that it is important to see an implant dentist (for an examination)as soon as possible if this problem is occuring, to find out the cause.

Q. I need an implant or possibly more.I have run out of tooth Upper right 6 and lower right 6 and 7.all very heavily filled! Im in Plymouth and need someone who specialises in implants.The list of dentists his site provides doesnt list the top specialists in the area.as a few names on the list are not dentists I would trust to do implants.Please help if you can.

Q. Hi there! I am a women and still young. I would just like to know can we the ordinary people on the street afford cosmetic dentistry and if you can do it through your medical aid. A. Unfortunately the NHS do not provide any kind of cosmetic treatment. NHS treatment is extremely basic and dentists that provide cosmetic and restorative dentistry have to have special training and should have better equipment and materials. Laboratory bills are also much more expensive for cosmetic treatment.

Q. I am an NHS patient who needs to have 2 implants in my lower jaw. I have a real problem in coping with any loose objects in my mouth. I understand that i will have to pay for the implants, but I am on a very low income. if i pay for the implant part am i entitled to have the crowns on the NHS ?
A. Under the NHS dentists cannot mix NHS and private dentistry on the same ” tooth ” – I presume this is the same for an implant.

Q. I have been suffering from a very bad odur coming from my two crowns for almost 4 years now. Three months ago one came up in a abscess for which root canal treatment was done. This seemed to eradicate the bad odur from that particular crown, however the bad smell still remains in the other. I am sure that there is some kind of infection however after an xray my dentist isnt sure. I am thinking about having a root canal treatment done on the other crown or possibly having both removed and bridged. This has really effected my life. Any suggestions would be welcome, thank you.
A. hi thanks for your enquiry, i think to have the two teeth removed is a drastic option. if your dentist is unsure about the second crown then ask him to refer you to an endodontist for a consultation, he will be able to assess your case with more experiance. hope this is helpful.

Q. Hi Rob, I grind my teeth badly and have for many years (apparently)i would like to replace four gaps for lovely implants, but how do i stop grinding first. I have a fifth tooth which half at the front has snapped off,made the tooth loose and developed an absess. Treatment did not work and it will have to be extracted. I do not want to go ahead with implants or bridges until i can find a way to stop grinding.
A. You may have read may previous replies which always bang on about sorting out the origional problems before starting treatments like implants. You have treat grinding by a combined approach, retraining habitual factors, and removing the prompts that that trigger the action. It is also important to have maintenance plan for lifetime care & monitoring.
