on an old root canal treated tooth and that no NHS

Q. Iam informed that I need an apicoectomy on an old root canal treated tooth and that no NHS Dentist can perform it also the local Dentistry Hospital can not perform it. Questions: Is the treatment really going to cost £500? What is the cost or is it a lottery? Where can I find a private dentist who is able to offer this specialist treatment without telephoning every entry of yellow pages? Why is it that an NHS dentist can’t perform the treatment but a private one can afterall they all have the same training ?

A. Dentists qualify with the same knowledge ( approx) but the training varies drammatically depending on how much investment they are willing to put into costly postgraduate training courses.  Due to the poor fees paid to NHS dentists per type of treatment they tend to have to keep their heads down and work incredibly hard.  Apicectomies are difficult to successfully carry out.  If I required one I would seek an endodontist to treat me.  Your local dentist should be able to refer you to a local endodontist  – the fee sounds about right for this type of intricate work ( truly microsurgery)

I have protuding front teeth with brown marks Ugh

Q. I have protuding front teeth with brown marks Ugh! I would love to look like a normal person and not be ashamed of my smile Where do I start ps I am 55 years old

A. In these types of problems the options open to you include:
1. bleaching and orthodontics to lighten your teeth and to move them into the correct position if possible
2. veneers to cover over the discoloured teeth and to reduce how much they protrude.

A. Hi
Thanks for your enquiry. You probably have the option for either fixed teeth or implant supported dentures, held securely in place with implants.

Fixed teeth are more expensive as more implants are needed and the laboratory work is usually more expensive. As a general guide an implant supported denture with 4-6 implants will cost from £7000. Fixed teeth need 6-8 implants and cost from £10,000.

I recently had a bridge made for the top of my mouth

Q. I recently had a bridge made for the top of my mouth, it was an eleven unit bridge supported by 5 teeth. The problem is the bridge has overbite also it is lopsided because my gums have recided on one side. This is the second bridge i have had made because the first had worse overbite and was too big and did not fit my mouth properly.I am now worried that the 5 teeth supporting the bridge (2 front, canine and 2 back teeth) will get too much stress and will have problems. Do you think i would be better just crowning them and going back to what i originally had so if problems did arise the teeth could be worked on. This has been going on 9 months now and i am getting very wary. I just want to go back to what i had, but my dentist says that having crowns on my front teeth wont last i will be better with the bridge, but the bridge is wrong and 11 units held by 5 teeth now seems wrong. Are their any strong porcelain crowns available and what is your opinion on how to proceed. I am pulling my hair out. Thank you

A. It maybe possible to replace the bridge with individual crowns and a chrome denture, however removing the bridge may weaken things further. You could also consider placing implants in the space to hold the missing teeth. this would reduce the stress on the remaining teeth and still provide you with fixed teeth. The final crowns could be made of either a strong porcelain known as Zirconium or porcelain fused to gold for extra strength. A. The costs varies from dentist to dentist.  With many of my patients I recommend a “denturist ” in my local area.  This usually keeps the costs down and the work can be completed while you wait.

Q. Which is the best crown on the market

A. It depends on what the situation is. In the anterior region a pressed ceramic crown is best. This doesn’t require the tooth to be drilled down as much as other crowns. It is a very natural looking crown and it adhesively bonds to the tooth so the reulting restotation is stron.
In the posterior section of the mouth the best crown is still a porcelain bonded to metal crown. Mant=y dentists would recommend a crown made of zirconia and porcelain. Although these are extremely strong there is a very high incidence of fracture of porcelain from these restorations and they can be very opaque.
If the tooth simply needs to be overlaid then a poraclin onlay would be the treatment option

I have had a bridge fitted on my top teeth

Q. I have had a bridge fitted on my top teeth due to the lateral incisor being missing on one side. I am not happy with the results as the 3 teeth now look bulky, stick out too much and are not a good colour match. I have been back to my cosmetic dentist who has said that it was difficult due to limited space because my canine had moved across a little to compensate for the gap. She has offered to do it again using a porcelain only bridge for a better colour match (& apparently less bulky), but she has warned me that the extra grinding required might kill the tooth and it would then need root canal treatment. She has quoted me £300 extra for a new bridge (plus £150 per tooth for any root canal treatment needed). Do you think opting for an all porcelain bridge is the best option for me now? And do you think it is reasonable for my dentist to request more money when I am unhappy with the initail result? Also, what are the risks of having a dead tooth? My teeth are currently very healthy. Thank you.

A. I would need to see the teeth as they are now, the pretreatment casts/photos/XRays to let you know what the best option is and the risks.
Liability for extra costs depends on what was promised before you started. In context  £300 is less than the lab fee I pay my technician for a three unit all ceramic bridge, and I would be also be losing my overheads. However we spend a fair time and effort predicting the result to the patient before we start treatment.

my veneers are yellow in between

Q. Hi ive had venneers done about 6 weeks now and in between each tooth is starting to go a yellow colour,i look after them using whitening toothpaste i dont smoke and have cut back drinking tea and coffee to about 2-3 cups a day! i dont drink coke etc!is there anything that i can do ?

A. You need to go back to your dentist and ask him/ her to make sure that you don’t have any glue inbetween the veneers because ths will stain. If the dentist tells you that it is the margin of the veneer then you haven’t had the best job done

Q. I have had fillings on the outside of my top four front teeth in the past. Would i be able to have teeth whitening done and would the fillings all have to be redone to match the colour and if so roughly how much would that all cost? cheers dave

A. The presence of sound  fillings does not prevent tooth whitening. Sometimes replacement is not required.
If you do need to replace fillings expect @ £100 but it can vary so a quote is important
tooth whitening with trays at home 350-500 laser whitening 4-800.

No, there is no escaping the dreaded drill!
A laser can be used once the nerve has been removed to sterilise the canal but the rest of the root canal treatment is done the conventional way

Q. i have been told that i am loosing the bone in the front of my two front teeth and that i will have to have them taken out i have already got crowns on the four front teeth what will the dentist replace them with as i dont want a great big gap in my front teeth

A. The qestion you need to ask your dentist is the reason for your bone loss and if it is gum disease why this hasn’t been picked up sooner.
If you have bone loss it may be impossible for you to have implants. I’m not sure what your dentist will replace your teeth with. Probaby a denture. You would have to ask him. If you do have gum disease then this needs to be treated before you loose any more teeth

A. hello,
we used to do this quite often but lately it is not as fashionable.there are 2 ways to do this one involves having the tooth drilled for a crown this is not advisable as is not reversible, cost  -400 onwards will be 80% gold.
the other method favoured by tv personaliites involves having a crown made that slides over your existing tooth without harming them and temporarily cmented so you can remove it as and when you like. f you use cheap metal or synthetic gold this will soon go black in the wet environment of the mouth, so i would advise a decent amont of gold cost 500 onwards.
there is no minimum age but i would advise 16 plus.

Q. hello dr. butt. i am going to be having all 4 wisdom teeth removed and am absolutely terrified. my dentist isnt the easiest person to speak to and a have afew concerns. they told me there is a risk of damaging a nerve that is in my lips and gum and in some cases the damage is permanent. what are the odds of that happening and what what it mean if that happened? i suppose i am after some more information on the procedure but now i am on the waiting list at my local hospital i do not have anyone to contact about it. i hope you can help, thanks in advance! 

A. dont worry  you will be assessed by the team at the hosital and all the different options with pros and cons will be explained then.
i would wait till they see you as they are used to addressing all the worries you are describing.

what is the cost of composite on a chipped tooth

Q. How much should I expect to pay for a bit of composite on a chipped tooth? My dentist seems to charge whatever goes and he never seems to have a clear cut pricing policy, although he is extremely good at what he does, hence he can charge whatever he likes! I’d just like to know what normal dentists charge. Thanks

THe answer does vary. We price composite on the the number of tooth surfaces it covers. this usually approximates to the time and difficulty. Some quote by estimating the time & difficulty for an appointment (this means that 2or more fillings at the same time will probably be cheaper than single fillings )
£80-140 gives some latitude

Q. over 30 years ago I broke my two front teeth and had crowns fitted one with a screw, then about 15 years ago had to have an apicectomy due to an infection and the crowns replaced. I was told that if I ever got another infection I would have to the crown removed and a denture put in its place, which has now happened. Is there any alternative to having a denture

There are several very good alternatives. The best option for you depends on the quality of the teeth adjacent to the space, and the bone volume.

Adhesive bridge; +ve Kind to supporting tooth -ve not suitable in some cases
Crown retained Bridge +ve You already have a crown next door. -ve more reliance on supporting tooth
Implant +ve stands alone, maintains bone volume -ve surgical procedure , longer treatment.

I have had a chip in my front left tooth

Q. I have had a chip in my front left tooth for about 10yrs. When i have my photo taken my teeth appear to look twisted due to the shadow it generates onto my other teeth. I am considering veneers but know nothing about them and i dont know if this will be the best option for me. please help me

Veneers are usually made of thin shell of porcelain that fits onto the front surface of the tooth.

Q. what is gum disease and what treatment can you have if you have gum disease

A. Gum disease usually refers to a problems with the supporting tissues that hold the teeth into the jawbone. Gingivitis refers to the upper level only Periodontal diseases result in loss of the attachment & bone around the tooth.
Basically you have to have bacteria/germs/bugs to cause the problem so the treatment involves thorough cleaning, removal of anything that hinders cleaning.
