I want 2 front teeth straightened with invsalign

Monday, June 30th, 2008

Q. I am looking to have my two front teeth straightened as they protrude slightly. I paid for an assessment for invisalign.html” target=”_self”>invisalign to be told I was in the \’severe category\’ and it would take two and a half years due to the position of my jaw, and the way my teeth fit together. I am now thinking of having crowns but seems very drastic. Are there any other alternatives, i.e. normal braces or would I have the same problems as with Invisalign?

A. It is very difficult to give you an exact answer without seeing you or some xrays of your mouth. If your teeth are very crowded then treatment can take longer. However there are other ways to straighten your teeth. It MAY be possible to reduce the orthodontic treatment by maybe slightly compromising the final outcome. For example if your only concern is your two front teeth and you are happy to live with the other teeth the way they are then this could reduce the treatment time. In my practice we treat a lot of severe cases but have not found active treatment lasting longer than 18 months.
I hope this helps

Botox injections for jaw pain

Monday, June 30th, 2008

Q. do you think botox injections would help my daughters jaw pain she as been to see consutants tried antidepresents and strong tablets but nothing as worked please help her she is in so much pain.

A. Botox works by paralysing the nerves in muscles and can reduce pain. However there are many other less invasive techniques to help your daughter. I would recommend a consultation with a dentist who treats patients with jaw problems. They will assess her and give you a number options to try. There are many types of appliances that can be put into the mouth which can help relax the jaw muscles and protect the jaw joints. These appliances act as a sort of physiotherapy. It would also be very important to assess your daughters teeth and the way they bite together.

I have failing tooth transplant and yellow teeth

Monday, June 30th, 2008

Q. Dear doctor my teeth are yellow through no fault of my own i dont drink or smoke.I have good oral hygiene put have a big gap due to a tooth transplant failing. Can you please tell me how much veneers are? and i have a false tooth where the transplant went wrong. I hate the look of my mouth the dentist said i have no more bone left to put a tooth in the gap. Maybe veneers will help the rest of the teeth look good thank you

A. I am sorry to hear your treatment did not work. I would strongly recommend a consultation with a good cosmetic dentist so your mouth can be examined and a number of different options explained to you. In my practice we see a lot of people who were told they could not have dental implants but there are great techniques where your bone can be regrown. It is a great option and may help you. Porcelain veneers are a lovely option to improve your smile but there are many other ones such as tooth whitening.

all my teeth are loose I have gum disease

Monday, June 30th, 2008

Q. I am 29 yrs old and have a bad gum disease and my i think all my teeth loose. What is my best for me to do at this point?

A. The first thing you need to do is go to your dentist or find one and have a full examination and consultation so they can assess the condition of your teeth and bones. Once this done they will be able to give you a list of options available and suitable treatment plans. If as it sounds you are suffering from periodontal (gum) disease then this is irreversible but it may be able to managed.

I hope this helps you.

I have a crooked smile and missing teeth can I get NHS treatment

Monday, June 30th, 2008

Q. Im interested in cosmetic treatment. I hate my teeth, i have a crooked smile. I have had braces but its my jaw that is uneven and im missing some teeth which makes my smile look strange. Is there any treatment i can get on the NHS? As i am a single parent currently studying and on income support.

A. I am afraid cosmetic dentistry is only available on a private basis and would be something you have to pay for. Some patients who come to see me who are on a limited budget apply for finance to help cover there costs. Our practice and many others provide 0% interest loans up to 2 years and low interest loans over longer periods. Some dentists who are starting out in cosmetic dentistry may offer you treatment at a lower price or even free if you attend courses with them so they can learn.

I have a dead tooth after root canal im in mubai

Monday, June 30th, 2008

Q. Hi Doctor I visited the dentist for a filling and during that my dentist told me that my upper left first incisor looked dead to her. When I was young I had braces and probably that did some damage. There is a light vertical crack mark on the tooth but i used to feet no pain or discomfort. My dentist took an x ray and performed an hot-cold test on the tooth and since I had no sensation in that tooth, she suggested that I root canal it to prevent damage to it and the neighboring teeth. After the root canal which was performed a year back, I am noticing that the teeth is turning dark — I am not too sure if I should get it crowned because it is the front tooth and I have heard people complaining that the white enamel on the crown sometimes just breaks. Is there any other way I can whiten the tooth? I also want to point out that when my dentist performed the root canal, I had no pain even when no anesthesia was used. So I also have doubts if the root canal at all was required. I am very worried. Please advise me. I shuttle between Mumbai and Delhi for work, and if you are aware of any good doctor in these cities I would be very obliged if you could pass me their contacts. In case age has anything to do with teeth, I would like to tell you that I am 25yrs old. Thanks

A. It is possible for a tooth’s nerve to die with no pain or symptoms. The darkening of your tooth is probably due to bacterial re infection of the tooth. I wouls advise you to have the tooth examined if the root canal treatment is judged adequate the first option would be INTERNAL TOOTH WHITENING. This is aprocess where the inside of the tooth (crown) is cleaned out and whitening peroxides are put inside. These can slowly lighten the tooth up again. Once the desired colour is reached a good bonded filling can seal the tooth. If this is not possible a crown could be an alternative.

I hope this helps you

My bridge feel uncomfortable and bulky

Monday, June 30th, 2008

Q. I have recently had a 3 tooth bridge fitted over three months ago. I am sure the dentist has fitted it incorrectly. It is very uncomfortable and feels bulky. The main problem is that on the inside of the bridge there is a lot of metal exposed under the gum where the porcelain does not cover, this feels like i have constant food stuck between the teeth and am constantly flossing to eliviate this sensation. My tongue rubs against the metal which is rough hence catching the side of my tongue and i have a constant metallic taste in my mouth. I am sure this is not right as i have a bridge on the adjacent side which i have had for 11 years with no problems whatsoever. My dentist sayes it is fine and I just need to get used to it. As 3 months have passed, and no improvement could you please give your opinion? Thank you

A. I am sorry to hear you are having problems. Obviously without seeing you or x-rays of your teeth it is hard to give you a definitive answer. The metal you feel at the back of the bridge is probably due to the fact there was very little tooth or space build the bridge out at the back. I think things ideally should have settled down by now. If the bridge does feel bulky it can be very unsettling as your tongue constantly rubs against it. I advise you speak to your dentist again to see if he/she will investigate it or can adjust the thickness to make it more comfortable. If things are no better it may be worth finding someone else to examine it and possible replace it.

I hope this helps you

My teeth are black and I don’t want a crown

Monday, June 30th, 2008

Q. I am 25 yrs lady. I had an accident and front 3 teeth got fractured .I had root canal done that time (10yrs back ) and teeth bonding done on 2 of the teeth . After 2-3 yrs all the three teeth started getting black. I did’t consult doctor after that .Few days back i went to doc and without checking anything he said that its better u go for Crown. I am not mentally prepared for crown because i don\’t want to grind my teeth. Is it ok if i would go for bonding again or veener ? Doctor says that crown keep the teeth strong. Is it the fact ? What is the problem if i go for Veener? Please suggest me something ..i really don\’t want to go for crown.

A. If the teeth are very dark then it may be that a veneer would not give you the cosmetic result you are looking for ie. There may be some show through of the underlying dark colour that a crown may prevent.

I would discuss your concerns with your dentist and he will let you know if it is possible to mask the colour via a veneer

what can go wrong with dental veneers

Monday, June 30th, 2008

Q. I have just visited a private dentist who told me that veneers would last 15-20years and cost about 7k, is this correct? After this time do the veneers fall off and what will my teeth look like then? Should I visit more than one dentist to gain a 2nd opinion? What can go wrong with this procedure.

A. The veneers would not come off in 15-20 yrs but may need to be renewed given that you will be biting and chewing for 15yrs it stands to reason that there may be some wear and tear.

I would always get a second opinion especially if you grind your teeth or suffer from headaches and jaw pain

Can I have veneers removed later and go back to my own teeth

Monday, June 30th, 2008

Q. I am thinking about getting some veneers fitted, however, I know that the life span of veneers is roughly 10 years. Once I’ve had veneers fitted, if one day I didn’t want them, could I have them removed permanently and go back to how I am now? Does the tooth preparation process to attach veneers affect the integrity of the tooth?

A. If you were to go ahead with veneers this is an irreversible procedure and you would not be able to take them off and be back to square one so you do need to assess the pros and cons of treatment and ensure you are happy with the quality of the treating dentists work

My back teeth are falling apart I want porcelain teeth

Sunday, June 29th, 2008

Q. i really need to have my teeth done, i am only 28 and the back ones are falling apart, my dentist wont do anything. Its not my fault that they are like this, i had problems and was put on a high dose of clexaine which messed them up. i want to have the porcelain teeth, i am worried about the price, but can pay monthly, i really need some help with having this done, teeth are so important, its the first thing you look at on a person, i am getting depressed over this, please help!!

A. You are right, your teeth are extremely important and have a huge impact on your face. I think that if your dentist won’t help you then it is time to find yourself a new dentist. He/she may not know what to do and this could be the cause for the reluctance.

If you have porcelain fillings to restore your teeth you need to go to a private dentist.

It is really important in your case that you go to someone that has a knowledge on occlusion. This is a study of how people bite together and how breakdown of teeth affect the rest of the functioning system. They will also have a knowledge of how to restore your teeth so that they will last and not breakdown.
If you are in Buckinghamshire or willing to travel I would be happy to see you for a consultation

I have a huge gap between my front teeth

Sunday, June 29th, 2008

Q. hello, i’m 18 years old and i have a really big gap in between my front two teeth, i measured the gap, and the bottom bit of the gap measures to be about 4mm, and as it goes up, the gap goes slightly closer together, with a gap narrowing to about 2-3mm. I have really low self esteem for the gap in me teeth, and was wondering, if veneers is the treatment for me? I would apply for invisalign.html” target=”_self”>Invisalign, but it\’s too expensive for my dad to pay! please help!

A. fixed braces may be an option if invisalign is too expensive. They cost around £3000. Veneers are definitely an option but to have it carried out properly and for the gap to be closed with the teeth in the correct proportion you need at least 8 veneers placed if not 10. If 2 or 4 veneers were placed you can imagine that the teeth would be very wide. There is a type of preparation procedure where we can make the teeth in proportion and make one the back teeth wider so that the front teeth still look good. This can however be more expensive than the invisalign.
Some dentists may offer to put white filling in between the 2 front teeth to close the gap. This will solve the problem of the gap but will give you 2 really wide front teeth instead.

In short, there are many ways to close the gap but if you want to have it done properly then unfortunately you can’t escape the cost. Given your age I think that fixed braces would be the best the option for you.
I’m sorry I couldn’t give you better news

How much will invisalign cost for a small gap

Sunday, June 29th, 2008

Q. On the top row of my teeth, I have a small gap in the front and a two small gaps one side. Roughly how much will it cost in total if I use invisalign?

A. invisalign.html” target=”_self”>Invisalign costs in the region of £5000. This is for upper and lower teeth. It may be less for just the upper teeth. It is best to have a consultation with an orthodontist to find out your options

I have removed my front top teeth

Sunday, June 29th, 2008

Q. I have had my two top front teeth removed ,what is the alternative to dentures,as i only have one tooth to the right of the gap.

A. Thank you for your question
The best thing to replace missing teeth are implants. You can have implants to replace individual teeth or you can have a couple of implants replacing 3 or 4 teeth. Implants can also be used to retain dentures. I advise that you have a consultation with an implant dentist to find out your options for implant treatment.

Am I too old for tooth whitening

Sunday, June 29th, 2008

Q. I am 62 years old and my teeth are very yellow can i still benifit from tooth whitening or what would you suggest

A. If your front teeth are not filled, you can definately have whitening and it will brighten your teeth. If you have large fillings in your teeth it may be best to have veneers if you want a long lasting results and strong teeth. The best thing to do is to find a restorative/ cosmetic dentist that is used to carrying out both types of procedures and they can advise you on the best course of action.

I need to replace my bridge for 3 teeth

Sunday, June 29th, 2008

Q. Hello Dr Suri I’ve had a bridge at the lower rear left side of my mouth for many years and it needs replacing. It covers the area of 3 teeth. I have changed dentist who is recommending instead of having a new bridge,to have two crowns and a dental implant for the missing tooth in the middle. Is this the way forward?! Which do you consider to be the best option? I’m healthy in my mid-30\s.

A. Your dentist is giving you the best advice. If I was in the same situation as you I would have two separate crowns and an implant to replace the missing tooth. Bridges ultimately fail because you have 2 teeth supporting the weight of 3. You will be faced with a the problem later on in life so it is best to take the pressure off the adjacent teeth now and have an implant placed.

My veneers are A1 shade but not white enough

Sunday, June 29th, 2008

Q. I recently had venners fitted to my front teeth, there not as white as i wanted them my dentist says there A1 but I know there not, he charged 145 per tooth which I thought was a good price. Can I have some more veneers put on top of the other ones so I get the colour I want, can you veneer on crowns as well Thanks

A. A1 is a natural colour but it does has a yellow undertone to it. It isn’t the whitest in the natural shade guide and there are porcelains that come in much lighter colours. Your dentist has probably made them in A1.
Veneers can be placed on top of existing ones and also ontop of crowns but it doesn’t normally end in a good aesthetic result. The teeth will look thicker than they should and will feel bulky.

It is your choice however, as long as you are realistic about the end results.
If you are really not happy with the colour and would pay for veneers to be placed ontop of the existing ones, I would have the old ones removed and new ones paced. Have it done properly and don’t cut corners because you may not be happy with the end result.

I am guessing that you went to a NHS dentist to have them done based on the price. when I have veneers made for my patients the lab bill alone is twice as much as the price you paid for your veneers. You have had an extremely good deal but I must also say that you get what you pay for and if you want a pleasing aesthetic result you will need to be prepared to pay more and go to a dentist that specialises in these procedures

I had lumineers on my front teeth now they smell

Sunday, June 29th, 2008

Q. I got Lumineers on my front 8 teeth, I am now having problems with my gums, they have a smell to them, I am always brushing and flossing them and it is not helping. Is this going to get worse? I never had this problem until they got put on. What do I do?

A. As a restorative/cosmetic dentist I cannot condem Lumineers if done properly. Great care needs to be taken when placing any type of veneers to make sure that gum health is maintained and is is at an optimum level during treatment.

Lumineers are a marketed very well and seem like the solution to all smile problems but they are only suitable for a small number of patients. Although it is good to be as minimally invasive as possible when preparing teeth for veneers and it is obviously best not to have any preparation at all for the teeth, there are many other factors that need to be taken into account when improving a persons smile.
We have to look at the health of the gums and whether they need to be reshaped, if they do we have to make sure that we are within the correct parameters for correction, otherwise it can create gum problems.

We have to take into account whether there are any fillings present in the teeth to be veneered. Veneers shouldn’t be placed over existing fillings because this can create reasons for failure of the restorations. When the filling fails and it will, so will the veneer. By definition, Lumineers are no prep veneers so I must assume that they are placed over existing old fillings.

If the teeth were initially crowded, then by having non prep veneers ledges are created on the teeth by the gingival margin which can act as plaque retention factors and cause inflammation of the gums. If the margin of the veneers have been placed underneath the gum to hide the ledge then the biological width may have been invaded. This is a vital area in the gum. It is an area of approximately 3mm from the edge of the gum to the bone holding the teeth in. It provides protection of the area and helps fight bacteria and is the natural defence of the gum. If it is invaded by the porcelain it can lead to chronic inflammation.

I can’t tell the cause of the inflammation without looking at your teeth and talking to you about your procedure. Inflammation can always be cured and gums can become healthy, it may however come at a cost.

If it is due to the invasion of the biological width by the veneers then either the lumineers can be removed the gums can be treated and new veneers can be placed. If you want to keep your Lumineers, some gum surgery can be carried out to resolve the inflammation. You will find that the gums shrink after the surgery as they become healthy.

If the inflammation is due to the ledges in the porcelain then they can be smoothed off and recontoured. The downside to this is that the porcelain is roughened and can be polished but is not the same as glazed porcelain previous to adjustment.

My advice would be to go and see a private dentist that is used to carrying out all kinds of cosmetic procedures. A dentist that performs veneer and restorative procedures and has a good portfolio of before and after pictures should know what he/she is doing. Make sure that the dental practice that you go to has a hygienist.

If you want to go back to the dentist that did your Lumineers then address some of the issues I have brought up in this reply and ask if there is anything that can be done. Definitely ask to be referred to his/ her hygienist.

If you do want a second opinion then I would be happy to see you for a consultation at my practice.
I hope this helps

I lost all my teeth and cant have implants due to bone loss

Sunday, June 29th, 2008

Q. i am 63 i l wear dentures as i lost all my teeth when i was 27 i dont think i would be able to have implants due to bone loss i read about stabilize ball anchor i would love to have a nice set of teeth can you give me any advice

A. It is always possible to have implants it just depends on the amount of treatment you are prepared to undergo. You may still have enough bone to place a couple of implants in the front section of your mouth and have attachments in a new denture that fix onto the implants. Your bones probably are quite receeded but there are bone graft procedures that can replace the lost bone if you don’t want to wear dentures at all. It may mean undergoing surgery under general anaesthetic but the end results would definitely be worth it.

If you are not inclined to undergo surgery then you can have a new set of dentures made, there are flexible acrylics available now that are thinner and new teeth could really give you the appearance that you want.
Have a consultation with an implant dentist to find out all of your options. If you live or can travel to Buckinghamshire I would be happy to see you for a consultation.

Will braces make my baby teeth fall out

Sunday, June 29th, 2008

Q. I have recently visited a private dentist for the first time to query braces/veneers. As I still have a lot of baby teeth in my top set I was informed that the brace may make them fall out-is this correct? If so would I just be left with gaps? He suggested veneers but wouldn’t this make the teeth fall out too?

A. The best treatment may be a combination of both types of treatment, braces and veneers. The choice depends on how many baby teeth you still have, whether the roots of the baby teeth are strong and whether the baby teeth are mobile or solid.
There is always a risk in both types of treatments that you may loose one or even some of your baby teeth and there is also a risk that your adult teeth need root canal treatment. This depends on how much your teeth need to be moved with braces or how much shaving down is necessary for veneers.

The other issue that needs to be taken into consideration is the way you bite together. If you have baby teeth I am assuming that the lower adult teeth touch them. This means that either way your treatment may need careful treatment planning and coordination especially if you are going to have combination treatment.
If you live or can travel to Buckinghamshire I would be happy to see you for a consultation to advise on the best course of treatment. My practice phone number is 01296 624163.

If you can’t then go and see a dentist that has experience in cosmetic and restorative procedures and someone that works closely with an orthodontist. Definitely don’t go to an NHS dentist as veneers are not available on the NHS and your treatment choices will be extremely limited.

Ask to look at a portfolio so that you can see before and after examples of the dentists work.
I hope this helps
