Manchester Invisalign on NHS or braces for students for front teeth sticking out

Q. I have very uneven teeth in particular my front upper four teeth stick out a lot, I am unhappy with the way they effect my smile and have asked my dentist about straightening them on the nhs, her response was that i was not to worry as its too late as i am a adult, i am a student and was wondering if there were braces or something that i could have done in order to fix them back in place and how costly it would be, i also have a cap on one of the four front top teeth and would like to know if the brace would move this tooth aswell, thanks.

I had a fixed brace them my wisdom teeth came through and now they are crooked

Q. I had a fixed brace as a teenager however my wisdom teeth grew in very soon after having my brace removed meaning that my bottom teeth are no longer straigth and my top front teeth have been pushed forward making them crooked, as well as my bite being completely offline. I am unsure what is would be the best and fastest treatment for me and would welcome any advice you could give. Thank you.

A. Hi H, this is the biggest problem with brace treatments, once the treatment is over many people don’t wear or are not given a retainer and therefore eventually the teeth which are in a beautiful position initially move back. The best option would be to have orthodontic (brace) treatment again whether that be fixed(train tracks) or removable(invisalign) treatment, but this time make sure you have a retainer for the maintenance of the treatment. This can take as little as 6 months to 18 months in normal mouths. Hope this is helpful.

I had 10 veneers due to tetracycline staining should i crown then now my gums are receding

Hello. I had 10 Veneers 19 years ago because of Tetracycline staining. One of the veneers came off and I ate it! But the others are ok and not chipped but the gums are receding a bit. Now I am looking for advice as to whether the next step is to Crown them. My dentist says it is. (Ideally I would like implants as they sound as if they would be for life! But the cost is too great.) Many thanks for your enquiry. well to start off with if your veneers have lasted over 19 years thats fantastic and there is no reason if by veneering the teeth again why they should not last a very similar time frame again. It all depends on how much tooth tissue is present and the more crucial thing how much enamel is left on the tooth once the old veneers have been removed. If there is a large quantity of enamel left i would probably re veneer the teeth, if the amount is less then i may decide upon crowns like your dentist has suggested. The new veneers or crowns would also help as would cover up the receeded gum areas. The advice on dental implants are that if your teeth are still in good condition then there is no reason to have these teeth removed and implants placed, this is a very last resort treatment for a mouth.

As i understand due to your past history with dentists i feel to cope with your nerves would be on two levels: explaining what is going on so you are less scared of the un known and also sedation to help you accept the treatment you want. Hope all this is helpful.

I have ugly crooked teeth and need braces in Ireland

Q. I am a 20 year old who needs braces badly. I have severly crooked teeth and not only do they look so ugly they cause me severe tooth aches. It is too expensive for me to get braces in ROI (Republic of Ireland) as I am the youngest of six children and there is no money left in our family. I would like to know what my other options are. My teeth trouble me physically and mentally. I never smile. Please help in anyway you can.

I am so sorry to hear your distress, it is very difficult to get orthodontic treatment as you older. A lot of private practices offer finance options for example ours does a two year 0 % interest and upto 5 years at low interest. This allows you you to spread the cost of any dentistry you want. As you say you are in pain I question whether orthodontics is the first treatment you should be having it would be important to get a full dental examination first as you may have some tooth decay, this would be needed to remedied before any orthodontics.

My jaw clicks and locks i cant eat solid foods and it hurts

Q. my jaw isnt straight and it clicks and locks most days- for the past few months i havent been able to eat solids without my jaw hurting so i\’d rather not eat. my old dentist said i\’d need my jaw broken and realigned, but my new dentist wont do anything. i\’m in constant pain and i dont know what to do. is there anything you can suggest? please!

Dear S, thank you for your question. You definitely need to be looked by someone that specialises in treating jaw problems. From your symptoms it sound like you have a displaced disc in your jaw joint. When your jaw clicks the disc in your joint hits against the boney part and this creates the noise. If your jaw is displaced to one side then it means that the disc in your joint is displaced and the muscle attached to the disc may be getting pressure from the boney part of the joint. It can be treated but without seeing you it is very difficult to recommend the best treatment. Don’t ever listen to anyone that says your jaw needs to be broken and reset. This will cause more problems, put you through a lot of trauma and will not work. In fact I don’t think that this treatment has ever been advoctaed for a displaced disc. I would need to go through all of symptoms with you and their onset and test and feel your jaw. In some instances splint therapy is the best way to go but in others in can be even more damaging which is why a case like this needs to be carefully looked and planned. A lot of dentists only have a basic knowledge of jaw problems and don’t deal with them on a regular basis.

If you live in Buckinghamshire or near my practice I would be happy to see you for a consultation. If you are in London then Dr Justin Glaister is the best person to go and see. He is also an expert on this site, or you could call the general dental council to find a list of restorative specialists in your area. If you take this option then be sure to ask whether they have experience with jaw problems.

In the mean time you should start taking 600mg of ibuprofen 3 times a day if you are not allergic or don’t have stomach ulcers. It is an anti inflammatory and will help relax the jaw muscles and the muscles that are attached to the disc.

When you sleep, make sure that only your forhead is resting on your pilllow and not your whole face this will prevent any unnecessary displacement of your lower jaw and I know this sounds silly and obvious but try not to open your jaw too wide. Also when you do try to eat make sure you take small mouthfulls and eat evenly on both sides. These suggestions are only to prevent aggrevation of your joint. They are not treatment methods. You do need to find some one other than the 2 dentists you’ve already seen.

Q. I have some teeth that arent straight but as i am an adult am reluctant to wear braces. Ive heard of invisalign and braces that go on the back of your teeth, whats best, how much will it cost and are there any practioceners in my area? many thanks

invisalign.html” title=”invisalign“>Invisalign braces are not fixed to your teeth. Moulds of your teeth are taken and the clear plastic removable brace is fabricated. This is then changed every few weeks as your teeth move. Length of treatment depends on how crowded your teeth are. Invisalign is not suitable for everyone. The other type of braces are lingual braces. These are fixed brackets that go on the backs of your teeth.
Unfortunately I cannot give you costs or treatment lengths as I have not seen your teeth. You will be looking in the region of £2000 to £3000.
The alternative to braces is of course veneers but this requires your teeth to be shaved down slightly and depending on the crowding can involve between 4 and 10 teeth, sometimes more. My advice would be to have a consultation with an orthodontist and with a dentist who places veneers, someone with experience so that you can make a decision knowing all the options

Q. My two front teeth are overlapping and one is slightly longer than the other. As a child I had years of braces to get them as they are now – they were a lot worse!! However I have never been happy with them and recently, after plucking up the courage, asked advice from my dentist as to whether veneers might be an option. He said that he wasn’t happy to fiddle with teeth that were healthy and that if I had veneers i might look like a rabbit. No other advice was given. Are there other options?

A. Dear G,

Thank you for your question. Unfortunately the only way to straighten out your teeth without braces is to have veneers placed on your teeth. There are options other than fixed braces to align your teeth such as invisalign.html” title=”invisalign“>invisalign, which is a removable plastic brace that fits over your teeth, these need to be changed every few weeks until the teeth are straight. It would be worth going to see an orthodontist for an opinion.
Usually when a dentist hasn’t had much experience in placing veneers they are reluctant to shave teeth down. It is true that having veneers done is destructive to your teeth and is an irreversile procedure. Therefore you need to speak to someone that knows what they are doing before you take the decision to have them. Veneers can be placed on teeth after very minimal preparation and if they are prepared by a dentist with experience and made by a laboratory that knows the correct techniques and characteristics of teeth (there are many labs that don’t) then your teeth will not only look good but the restorations will be long lasting and life like.

It is your decision at the end of the day. If you decide to have veneers then please find someone that knows what they are doing. Ask to see a portfolio and ask the dentist about the laboratory that is used. Most good labs have their own website. This way you can make an informed decision.

I have turned 40 and need to do something about my smile

I’ve recently turned 40 – yuk – and decided to do something about my smile. I have a crown on the tooth next to my upper canine which is now more yellow than white and two veneers on the bottom front centre teeth which “wrap” over the top of the teeth to make them slightly longer, similarly they are slightly discoloured. I have previously whitened my teeth which looked wonderful except for the crown and veneers! I have since then tried to “top up” the effect because I am a smoker and drink a lot of tea and over the years my teeth have lost the whiteness again.

I was told to re-do the home whitening and did so – this time I developed some white spots on two teeth, a recent visit to a different dentist revealed that the home system I was using is fairly ineffectual and old technology – I had to use it overnight and my teeth had become sensitized. I am now using the home system by Zoom called “Day White” at a 9.5% strength for an hour a day to try and restore the former whiteness.

My two top centre teeth have had composite work done to fill an existing gap when I was 20 and the mid-line is slightly off. I would like a brighter smile and whiter teeth and after much research believe that veneers are the best way to achieve this (obviously I also need to replace the existing crown). I’ve been interested to read about your “one-day” onsite smile makeover and although I can see the benefit in having an onsite technician who can exactly match the veneers to the patient, am interested to know whether veneers created in this way are as good, quality and durability wise, as the more usual veneers one has to wait a number of weeks for. I was recently told that German and American labs produce far better quality – is this true? L

 

A. Hi L,

many thanks for your enquiry, ok lets tackle each concern separately.

teeth whitening“>Teeth whitening is very dependent upon the concentration of the solution used and how long you have the solution on your teeth. Also new research suggests that the type of whitening trays have a part to play on how well the treatment works. So to summarise the longer you have the solution on your teeth everyday and the more often you use the whitening treatment ie 1 daily to twice daily you will see better and better result. So perseverance is the best advise. The best home treatment on the market is however a system which encompasses all these variables to produce a excellent result called enlighten. Also while this treatment is being done try to avoid smoking and drinking and eating foods of high colour.

Veneers are a very good option in correcting things like poor colour, shape, contour and midline correction of teeth. There are many types of veneers on the market and the basic things to understand are that some veneers are built up by adding porcelain firing it then adding some more and firing it until the final produce is produced (time consuming). The second way is a pressed veneer which is built up in wax first then the wax replica is made into porcelain by a pressing process(empress veneers) this produces a very consistent result and due to the core porcelain colours being limited can be reproduced much easier, then there is milled porcelain veneers (CEREC), which is ground from a single block of porcelain. All these veneers in the hands of the right technician can produce beautiful results and equally all these three types of veneers should last equally well.

There are good and bad technicians in all countries. I use a fantastic technician in the UK from a dental laboratory called ART, Gary Jenkinson, who produces work which is second to none, so it’s all a matter of having the right team and the technician being willing to learn.

I want to change all my front teeth to silver colour dental grills?

Q. I want to change my canines and all my front teeth to silvery colour teeth, please can it be possible?

A. Hi S, many thanks for your enquiry, to change your front teeth to a silver colour is what i am understanding that you require.

There are two ways to achieve this. the first way is to have a clip on device called a grill made for your teeth which is made to the specification you require. This is removable and so if ever you don’t want silver teeth anymore is not a major problem.

The second option would be to veneer your front teeth but instead of the veneers being made out of porcelain would be constructed out of a silver alloy. This is possible and is a fixed option so if ever you decided you did not want silver teeth you would have to have the work removed and replaced with an alternative ie, porcelain veneers. To create the room for the silver veneers the teeth would need to be prepared so drilled.

I had Root canal and need a crown how long will a crown last

Q. Hi Dr Solanki, I had root canal treatment”>root canal treatment on one of my teeth over a year ago, the tooth has now broken. I have been informed by my dentist that I now need a crown as soon as possible. Could you please advise me what crown I should go for and how long one would last. Also, please could you tell me how much I should expect to pay for a crown as i’m looking to change dentists, and want to make sure that I choose the best one. One more thing, what are the chances of a crown falling off? because the last thing I want to do is pay for the treatment and within a month it falls off. Many thanks in advance.

A. Hi R,

Many thanks for your enquiry. this is a common problem i see from day to day. Once a tooth has been root canal treated the crown portion of the tooth is generally weaker and subject to a lot of biting forces. In normal function when a tooth is alive, if you bite hard upon the tooth the normal reaction is for the tooth to feel the force and subconsciously say “i don’t think i am going to do that again”. Unfortunately when you root canal treat a tooth it loses the feeling in the tooth so if you do bite hard upon the tooth it can be destroyed. This is why a crown is always the best treatment option to help protect the tooth.

Which type of crown to chose is then the next option. The most common type of crown is a metal ceramic (bonded) crown, this has metal on the inside of the crown to give it strength and porcelain on the outside of the crown to mimic tooth colour. These crowns if done well can produce beautiful aesthetics but the technician has to be very talented. The main disadvantage of this type of crown is that they are generally finished at the join where the crown meets the tooth by metal so when the gum around the crown recedes’ the first thing you see is this dark metal collar around the tooth which looks unsightly.

The more widely used crown in modern dentistry for long term aesthetics in the front parts of the mouth are all porcelain crowns. This means the crown is made from porcelain and has no metal sub core, this also means that when the gums recede you do not see a metal collar around the crown. Due to the problem of root canal treated teeth darkening i would recommend a lava or procera crown which can be made to also block out the colour of the tooth underneath.

The cost of crowns vary from area to area and this also varies dependant upon the technician doing the work. The average cost of a metal ceramic crown ranges from £450- £900, and all porcelain crowns range from £600- £1500. Hope this is helpful.

Q. I already have sensitive teeth but i really need to whiten them. What do i do with regards to the sensitivity as i hear it gets worse afterwards. Have tried so many tooth pastes for sensitive teeth but they don’t seem to work. Can i go ahead and whiten even with the sensitivity?

A. Dear R,

During the whitening process your teeth are likely to be more sensitive, some people grin and bear it as it is only short term. The other option is that you can pre treat your teeth before whitening with a fluoride/ desensitising agent this reduces sensitivity during the process. However, first you must find out why your teeth are sensitive and treat that cause first. It could be due to periodontal / gum disease or due to toothwear.

I have a crooked smile and want a quick solution as I am getting married

Q. Hi, I have over-crowding in my top teeth (and lack of space for movement) which has resulted in a crooked smile. The front teeth are fine, its just the ones that are next to them. I want to get married soon and I’m looking for a quick soultion. I’ve previously started treatment but honestly do not like the dentistry experience!!

A. Hi H,

For a quick fix it is likely that Porcelain Veneers are going to be a quick solution that looks good and lasts, direct composite veneers are cheaper but will need replacing after a few years. Usually crowded cases require veneering of the front six teeth because you need to “ borrow “ space from one area to the next. It is not a painful, sometimes no drilling at all is needed. Some cases can be completed in two treatment appointments of about 1 hour. You would need to have a detailed assessment before treatment to show you what your end result would be.

I have mild over crowding and want to straighten one tooth

Q. Hi, I have mildly overcrowded teeth on my upper and lower jaw. One of my lateral incisors (the tooth next my right front tooth) has become crooked and partly pushed behind my right central incisor (front tooth). In photos this gives the appearance that I have a gap in my teeth and is making me feel self conscious about my smile. I don\’t want braces and just want to straighten this one tooth out – or improve it’s appearance in some way. I’m totally happy with the rest of my teeth. Could veneers help me or is the only way to go braces? Do you have any idea how much treating this one tooth might cost? Thanks for your time!

A. Hi E,

Thank you for your question,

Without looking at the your teeth it is difficult to tell you for sure that a veneer could improve the way things would look. Braces are always the best and least destructive way of correcting crowding. Other alignment problems can also be corrected in your mouth ensuring that your smile is healthy and aesthetically pleasing but the treatment can take between 18 months to 2 years. If you are reluctant to go down this path then the other option is to place a veneer. This may not look quite right however because there is a reduced space between your central incisor and canine. This means that the resulting restoration will be narrower than the one on the other side and therefore they will be symmetry problems. This may be a compromise that you are willing to accept if you don’t want to have orthodontic treatment. The other consideration to make is that if a veneer is placed on the tooth without having any tooth preparation then you will end up with a tooth that is thicker than your other and still may not be in proper alignment. If the tooth is shaved down slightly so that the tooth is not thicker and is in proper alignment you will be compromising the health of the tooth. The lateral incisor is the smallest tooth in the mouth and therefore even if a conservative prepartion is done, as a percentage of the tooth, it may be substantial.

I think the best way to decide is to have your dentist place a composite veneer on the tooth. This is a white filling material and can be placed without damaging the tooth. It will be like a mock veneer. You could live with it for a while and see if you are happy with the result. It is completely reversible so you won’t have compromised anything, only a little time.

If it were my tooth I would probably have braces but everyone is different and so I think the composite veneer is a good way to go so that you can go into any treatment braced with more knowledge. (pun not intended)!

regarding cost. This can vary greatly depending on the experience and training of the dentist and the laboratory used to fabricate the veneer. Veneers can range between £450 and £1200 at the top range. Make sure that you ask the dentist what kind of experience he/she has and ask them to show you examples of their work. If they are doing this kind of work regularly they will definitely have a portfolio. This a part of your body and you have to have someone that you trust and knows what they are doing. To Give you an idea I would first place the composite veneer and see how you liked the look of it. If you decided to go ahead with treatment I would have the lab that was going to make the veneer see you to make sure that the shade is matched exactly to the teeth on either side of the gap. Once the preparation is done a temporary veneer would be placed until your final one is ready.

can you bleach a dead discoloured single tooth?

Q. Dear Dr. Patel Two years ago on holiday i cracked my canine tooth on popcorn and unfortunately the nerve died. On return from my holiday the tooth was assessed and root canal treatment was performed. My dentist felt that the crack wasn’t bad enough for the tooth to be crowned and commented that the canine tooth was a strong tooth so it should be fine. As the tooth has died it is a couple of shades darker than the rest of my teeth and i would like to ask your advice if a single dead tooth could be bleached. What would my options involve. When a tooth has root canal“>root canal treatment it can darken with time. This is often due to bacterial waste products still present in the tooth. The first thing I would recommend is assessment of the root canal filling and the filling sealing the tooth. Once these are okay a process call internal tooth whitening can be carried out to whiten the tooth. The other options would be a porcelain veneer or crown to cover the tooth.

Q. hi, im 20 yrs old, i had a brace put on when i was 16 to move one tooth… and i actually wanted more done with the brace but the dentist brushed it off and said that will happen in time… however when the brace was taken off the one particular tooth that he wanted to move slightly moved but now its gone bk to how it was so basically i had a brace on for no reason… anyways i was wondering what the price range is for invisalign treatment and if it can make my smile wider? because some of my back teeth are in the right place but they go narrow near to the front, will invisalign work? thanx

composite veneers or porcleain veneers on bottom teeth?

Q. I had some porcelain veneers done 4yrs ago on my top front teeth, which still look like new now. I went to see my new dentist at same practice to enquire about porcelain veneers for bottom teeth, but was informed that I wouldn’t be able to have them so was told I would have to have composite veneers. I had them done just prior to xmas 2007 and already the composite on 2 are badly worn. I was informed procedure would take approx 30mins per tooth, yet I had 6 done in 90mins! I would appreciate your views and advice on this matter. Have I been misinformed? If so, is there anything I can do about it?

A. Hi A, thank you for your question.
It is difficult to understand why veneers couldn’t be done on your lower teeth. If they are suitable for composite veneers they should really be suitable for porcelain veneers.
Porcelain is much harder and more abraisive than composite, therfore composite doesn’t last as long but when it is put against porcelain it wears even faster. If the composite composites veneers are too long or are too thick they will grind against the upper porcelain veneers during chewing and can wear down even further.
Everybody has different techniques to fabricate composite veneers and can it can take anywhere from 30 mins to 4-5 hours. The best material to put against porcelain is porcelain. I think it is definately worth seeking a second opinion. If you live close to buckinghamshire I would be happy to see you for a consultation.

cost of my dental treatment £900 per tooth

Q. Dear Dr Glaister,

Much appreciation and thanks for getting back.

I have had some treatment in that the UL1 has been prepped with a temporary veneer and UR an UL 3 have both been cosmetically bonded (contoured). People have noticed a big difference already. The only thing that scares me is this dentist filed my UL1 down to a pin for preparaion without explaining everything. I have a review this week before commencing any more treatment but I was shocked at how much tooth was removed and I was shaking at the end of this session.

He advised he could not give me a treatment plan or full costings although I am being given prices for each stage.Their website quotes £395, the receptionist told me £600,the dentist said £800 and now the practice manager tells me £900 per tooth.

I notice that this dentist does appear it the top ten UK cosmetic dentists lists and I have seen examples of his work on another dentist and the results do look great. It’s just that I don’t fancy having all my teeth filed down to pins as the majority of them are healthy.
